Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting and implementation of care plans. Makes referrals to outside agencies and coordinates continuity of care. Works collaboratively with the multidisciplinary team to optimize patient outcomes. Contributes to the attainment of organizational goals. Will be required to provide coverage during absences, vacancies and on call.

Johns Hopkins Hopital in Baltimore, MD is hiring for a Social Worker in the Pediatric Outpatient Cardiology.

Schedule: 

  • 40 hours per week
  • Monday- Friday 8:30AM - 5:00PM

Responsibilities

  • Completes psychosocial evaluations on assigned patients meeting the institutions' compliance standard of 72 hours. Displays good interviewing, diagnostic, and report writing skills. Conducts therapies in accordance with patient diagnosis and needs with appropriate supervision.
  • Provides efficient discharge planning throughout the treatment planning process and according to specific assignment. Identifies patients with complex discharge planning needs. Works collaboratively with physician, patient, family members, and payers as appropriate to develop discharge plan.
  • Completes progress notes and assessments according to program procedure.
  • Assists in developing and formulating the individualized treatment plans for each patient and ensuring each patient is aware of such.
  • Conducts individual psychotherapy sessions in accordance with state/regulatory standards, displaying good theoretical knowledge of group therapy principles and good group therapy technique.
  •  Engages in timely, efficient discharge planning. Works in close conjunction with patient, family, nursing staff, and attending physician. Shows good knowledge of referral/placement process and available community resources.
  • Completes progress notes and discharge plans in a timely, professional, high quality fashion.
  • Assess, plan, implement, and evaluate behavioral conditions requiring crisis intervention services.
  • Conduct behavioral assessments using formalized screening tools or protocols.
  • Formulate appropriate treatment recommendations incorporating the least restrictive approach. Communicate effectively regarding patients to treating ED staff, psychiatrist-on-call, and other involved individuals in the hospital and community.
  • Integrate current research into clinical practice at all levels. Promotes the utilization of current research by all staff members as a guide to daily practice.
  • Interact with the clinical team to advance the quality of care.

SKILLS AND ABILITIES REQUIRED:

  • Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting and implementation of care plans. 

  • Makes referrals to outside agencies and coordinates continuity of care. 

  • Works collaboratively with and provides leadership with the multidisciplinary team to optimize patient outcomes. 

  • Serves as a clinical resource expert in a specialty area.

  • Must focus in the area of clinical practice, education or research. 

  • Functions as a mentor for others.

Qualifications

  • MSW degree from an accredited school of social work.
  • Must be licensed by the Maryland Board of Social Work Examiners at the LMSW level or LCSW-C.
  • Minimum of 1 year experience in providing clinical social work service in a hospital - preferrably in a pediatric unit.
