This position is a split position between administration and caregiving responsibilities. 

Benefits:

  • Paid Vacation: 14 days of paid vacation annually, eligible after 6 months of FT work. 
  • Paid Sick Leave: 1 hour of paid sick time off per week. 
  • Benefits-eligible after 3 months FT work: Medical insurance, 401K. 

Schedule:

  • Office Hours: Monday- Friday 0800-1630.
  • This position is approximately 50/50 in the field and the office: Hours in the field are variable.
  • On-Call Caregiving Requirement. Must be willing to travel to client's homes for shifts.

Holidays:

New Year’s Day (Jan 1), Martin Luther King Jr. Day (third Monday of January), Presidents Day (third Monday of February), Memorial Day (last Monday or May), Juneteenth (June 19th), Independence Day (July 4th), Labor Day (first Monday of September), Veteran’s Day (Nov 11th), Thanksgiving Day (fourth Thursday in November), Native American Heritage Day (Friday immediately following fourth Thursday in November), Christmas Eve (December 24th), Christmas Day (December 25), The evening of New Year’s Eve (Dec 31, 6 pm-midnight).

* Holidays are scheduled off with the exception of on-call/backup care requirements. * 

Essential Job Functions:

  • Manage the Home Care Department: Ensure all Home Care Plans of Care (HCPOC) are up to date, manage a team of caregivers, oversee and conduct on-site supervisory visits in the field, assist with the management of client's schedules, other duties as assigned.
  • Team Building: Manage, support, lead, and train caregivers in the field. This position is hybrid in the office and clients' homes. You will manage and oversee a team of caregivers, ensuring proper care for our clients at all times. 
  • Case Management and oversight: Assist in patient admission process/ hands on patient care at place of assignment. Requires frequent visit to client homes; with the completion of plan of care updates, training for staff, compliance requirements, skill nursing visits, and shift coverage as needed.
  • Scheduler Assistance: Work with our scheduler to ensure coverage at client's homes and that all necessary orientations and nurse delegations are in place for caregivers first shift. 
  • Education: Backup trainer for offered courses.
  • Fosters a high-performing culture of excellence that drives employee engagement and provides exceptionally reliable, safe, high-quality patient care.
  • Perform other job-related duties as needed.
  • Assist with administrative tasks, record keeping, and documentation. 

Caregiving Skills: 

  • Perform hands-on patient care at the place of assignment: In-home, Facility, or Adult Family Home.
  • Provides quality care to patients/clients in an environment which promotes patient/ client rights, dignity, freedom of choice, and individuality.
  • Provide individualized attention and encourage residents to maintain or attain the highest practical physical, mental, and psychosocial well-being.
  • Understanding and ensuring that individualized care plans are followed. Report any change of condition or observation of patient need to nurse supervisor.
  • Assist with ADLs: including assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals’ care requirements as directed.
  • Promote skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning, repositioning immobile residents, and applying moisturizers to fragile skin.
  • Treat each patient client's personal property with respect.
  • Performs various delegated tasks assigned by the nurse including vital signs, residents’ weights, and applying creams/ointments.
  • Physical able to lift 50 lbs, using proper body mechanics, or lifting devices for accident prevention.
  • Understand and be able to follow safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.

Qualifications/Educational Requirements:

  • High school graduate or G.E.D certificate.
  • CNA or HCA license to provide home care in Washington State.
  • At least 1-year experience as a Home Care Aide (in homes or facilities), or other applicable experience.
  • Technology Savvy.
  • Understanding of Microsoft Office: Word, outlook, Excel. 
  • Be listed in good standing with the Washington DSHS Background Check System.
  • Ability to read, write, speak, and understand English as needed for the job.
  • Have transportation to and from job.
  • Valid driver's license and insurance.
  • Population-specific experience a plus, (training may be provided) for conditions such as: dementia, Parkinson's, hospice/end of life care.
  • RN delegation certificate a plus. May complete training through Journey Nursing Services (via Journey University).
