General Purpose

The clinical partner will provide/maintain the highest quality of nursing care support for Assisted Living members utilizing current knowledge and standards of practice. Central to this practice, the Clinical Partner will maintain the Members independence by encouraging their active participation in decision making and plans of care. The clinical partner will build a relationship with every member based on trust and respect. All care performed will be done in such way to uphold the members’ dignity, to create a supportive environment fostering a sense of home and community, and to add to the enrichment of each member’s life. As a member of the Crane House team, the clinical partner must demonstrate effective interpersonal and interdepartmental communication skills. When necessary the Clinical Partner will provide any necessary support to Care Partners establishing a harmonious work environment and assuring a high quality of care.


Essential Duties and Responsibilities:

  • Seek to understand individual member’s preferred routine and unique wants/needs.
  • Maintain independence and a sense of control by encouraging each Members participation and decision making in the care planning process.
  • Maintain each member’s self respect and dignity at all times.
  • Provide members with quality nursing care along with meaningful interaction and socialization.
  • Assist in the development of each member’s unique care plan in collaboration with the assisted living team, members, families, and physicians.
  • Actively listen to member and family concerns and provide feedback in a compassionate and professional manner.
  • Provide caring, emotional support to members and families during difficult times.
  • Document any changes in a member’s health or ability to perform daily activities and seek guidance or assistance from appropriate clinical and administrative staff as needed.
  • Provide recommendations related to changes in member care needs and communicate all level of care changes to the assisted living team.
  • Provide a thorough assessment and complete the appropriate documentation of any accident or incident with a member or employee in Assisted Living.
  • Whenever possible, assist with clinical care and/or assessment completing the appropriate documentation of an Independent Living member.
  • Teach and mentor care partners to continuously improve member care.
  • Whenever possible support the care partners in providing member care needs regardless of the task required


Behavioral Skills:

  • Enjoys caring for older adults with the goal of maintaining the person’s independence and keeping the care specific to each member.
  • Takes pleasure in socializing with older adults while providing nursing care.
  • Demonstrates a high level of empathy and understanding with all Members with a particular focus on those with cognitive and/or physical impairments.
  • Exemplifies the highest level of customer service as evidenced by: greeting members and visitors with a smile and positive attitude; calls members by Mr. or Mrs.; uses a member’s first name only if it has been requested; knocks on a member’s door and waits for a response before entering; and goes above and beyond to respond to those “spur of the moment” requests of members.
  • Provides honest and open communication with members, families, and coworkers.
  • Works as a team player within Assisted Living and all other departments.
  • Willing to pitch in and assist the Care Partners/Agency personnel with their duties as needed.
  • Attention to quality and detail of work such as in the regular assessment of members to monitor minor changes in health or ability.
  • Highest regard for personal appearance: uniform, hair, and nails are neat and clean.
  • Strong communication, oral, written, and listening skills.


Minimum Qualifications, Education and Certifications:

  • Graduate of an accredited nursing school with an active nursing license in the state of Ohio.
  • Current knowledge of state and federal rules and regulations related to long-term care.
  • 3 years clinical experience in long-term care preferred.

Physical demands may include walking, talking, hearing, kneeling, crouching, stooping, reaching, pushing, climbing a ladder, manipulating, standing for extended periods, lifting up to 75 pounds, transferring resident up to 150 pounds with assistance, and repetitive motion.

Every Monday and Friday 7pm to 7:30am
