Position Summary

The Residential Case Manager is the advocate for assigned children in our residential program with the responsibility to assess needs, coordinate resources and assist with adjustment and inclusion in out-of-home placement. This position is responsible to review and maintain accurate records, develop the Individual Service Plan as well as monitor and facilitate goals for each child, provide status updates and collaborate with guardians, family members, and other staff for the duration of placement, and properly handle discharge and aftercare for the child. The Case Manager should maintain positive and professional communication with our children, Agency and program staff members, community partners, and stakeholders in order to carry out our mission.

Primary Responsibilities and Major Objectives

  • Secure all required documentation for a child’s acceptance and admission to American Children's Home, before or at the time of arrival. Maintain updated record information as defined by the program following intake.
  • Responsible for the Individual Service Plan, reviews, record keeping, and case management of each child assigned to the residential program.
  • Communicate with program staff to ensure that all children assigned have adequate clothing, personal care items, bedding, and school supplies and assist in making purchases or procuring items as needed.
  • Initiate each child’s school enrollment, attend school conferences as needed, collaborate with teachers and school liaison and monitor student progress.
  • Ensure each child has access to counseling. Provide information for mental health intakes and schedule appointments as required by state and accreditation requirements. Maintain contact with other professionals that provide services to the child and family.
  • Identify needs and interventions of children that are age/developmentally appropriate and coordinate services.
  • Provide weekly one-on-one time with each child assigned to focus on Service Plan goals.
  • Demonstrate knowledge and apply skills in a manner consistent with the CARE model.
  • Respect the ethnic, cultural heritage and family traditions of children in care.
  • Strive to build and maintain a therapeutic alliance with each child assigned in the program.
  • Communicate with other program staff about concerns, needs, Service Plan goals, or other areas of opportunity that are in the best interest of the child.
  • Provide information that will assist in reducing interpersonal conflict and making all children and staff feel safe in their respective programs.
  • Serve as a resource person to provide information, ideas, and training for children, families and staff.
  • Attend professional meetings such as court and child/family meetings with DSS.
  • Manage workload efficiently (including, but not limited to, filing, documentation, measuring outcomes, and record keeping) to meet accreditation and state standards.
  • Willingness to participate in professional development opportunities by attending training, seminars, and in-services that are related to this position or as assigned.
  • Participate in the Agency's Performance Quality Improvement (PQI) program and committee meetings as needed.
  • Attend fund-raisers, group outings, or other activities that may occur on off time.
  • Perform other duties as assigned by Case Manager Supervisor.

Minimum Qualifications:

  • MSW or BSW; or a four –year degree in a related human service field and two years of previous experience.
  • Acceptable driving history and a valid driver’s license.
  • Must not have a child abuse substantiation.

Physical Demands:

  • Frequently required to stand, walk, sit, and climb stairs

  • Occasionally must lift and/or move up to 35 pounds, frequently lift and/or move up to 10 pounds
  • Specific visual abilities required include close vision; distance vision; peripheral vision; depth perception and ability to adjust focus; vision for driving
  • Occasionally exposed to outdoor weather conditions
