Kadlec is Hiring a Care Manager MSW - Clinical Resource Management Near Richland, WA

DescriptionThe Care Manager MSW partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The Care Manager MSW is responsible for providing a full range of social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The Care Manager MSW partners with the patient and his or her support system, as well as interdisciplinary teams, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of health care resources. The Care Manager MSW assumes a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes. The Care Manager MSW is guided by the NASW Code of Ethics and the Kadlec Mission and Vision in his or her daily practice.

Required Qualifications
  • Master's Degree In Social Work or related field from an accredited educational institution.
Preferred Qualifications
  • 1 year Recent inpatient clinical experience in an acute care facility.
  • 1 year Brief intervention/therapy/counseling in a hospital setting
