Clinical Care Manager of Post Transition Services
POSITION IS ELIGIBLE FOR HIRING BONUS
Due to eligible grant funds, this position offers a $2,000 hiring bonus. The bonus will be paid to the hired candidate half after 30 days ($1,000) and half upon the successful completion of 6 months of work ($1,000). Employees must be active and in good standing at the time of bonus payout to be eligible for hiring bonuses.

Job Scope: The Post Transition Clinical Care Manager provides direct-service and clinical oversight to Colbert and Williams class members that have transitioned out of nursing facilities . Provide assessment, care coordination, and clinical community-based interventions for mental health and/or physical health needs. Create comprehensive wraparound service plans and coordinate and monitor services. The Post Transition Clinical Care Manager will oversee and work collaboratively to coordinate services with KYC CST/ACT Teams as assigned by class member. Treatment services are delivered at flexible times that meet clients’ needs and in settings that offer the best potential for positive outcomes. The Post Transition Clinical Care Manager must have a Master’s Degree in social work or counseling, and have a valid Illinois Driver’s License and have ability to supervise assigned intern
Program/Clinical Responsibilities 
  • Meet regularly with Class Members in the community in the KYC service area around the Northwest Suburbs of Chicago 
  • Oversee and coordinate to ensure quality and access to all recommended services for Colbert and Williams Class Members that have transitioned out of nursing facilities and are assigned to KYC CST/ACT treatment teams 
  • Coordinate with the Clinical Care Managers throughout the discharge planning process for Class Members in the pre-transition stage to ensure continuity of care after nursing home discharge 
  • Take initiative and be accountable for and manage all aspects of post-transition, including being able to navigate changes at the system or individual level and advocate for Class Members and resolve conflicts. 
  • Review transition plan to ensure all aspects are being implemented 
  • Provide comprehensive, integrated service plans and connect to services to help clients improve, regain, and develop skills needed for daily life focused on community mobility and integration, cognitive processes, support with engagement in IADLs/ADLs, hygiene and self-care, sensory processing, safety with transfers, fall prevention, and adaptive equipment to increase safety and independence 
  • Create comprehensive wraparound service plans for class members, using input from Class Members, SMHRF/NF staff, family and guardians while focusing on a Class Members needs, desires, and preferences that integrates all facets of clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, spiritual, community-integration, and housing needs 
  • Communicate clearly with the Colbert/Williams interdisciplinary transition team, assigned ACT/CST mental health treatment team, Class Member and family/primary supports, and various treatment providers to ensure working together on service goals 
  • Provide post-transition monitoring to Class Members for 18 months and meet minimum in-person visit schedule to ensure all services are being accessed and delivered 
Administrative Responsibilities 
  • Identify and complete written reports to IDHS on reportable incidents involving Class Members post-transition within timeframe 
  • Participate in weekly team meetings in order to coordinate and support post-transition efforts 
  • Complete all clinical documentation and required reports in an accurate and timely manner
  • Interpret and communicate, at the service delivery level, Center policies and procedures 
  • Participate in orientations, all staff trainings, and training and development seminars
  • Submit, in a timely and accurate fashion, schedules timesheets, mileage and other reports required by the Center’s policies and procedures 
  • Coordinate closely with Supervisor of Clinical Care Services 
Minimum Position Requirements  
  • Masters Degree from an accredited College or University in Social Work, Psychology, Counseling or closely related discipline 
  • Qualified Mental Health Professional (QMHP) under Illinois Rule 132/140
  • Preferred LCSW or LCPC, or eligible in next 6 months 
  • Preferred experience working with adults with serious mental illness and experience working in non-traditional treatment environments i.e.in-home, residential, nursing facility, hospital, community settings 
  • Applicant must have a valid IL Driver’s License, proof of insurance, and daily access to a reliable well maintained vehicle 
  • Computer and smartphone proficiency, including working knowledge of MS Word, Excel, Internet skills, text messaging and electronic health record. Knowledge and ability to learn and effectively utilize electronic internal collaboration and communication tools (ex. Microsoft Teams, Microsoft 365) 
Schedule 
  • Full-time, Salary, Exempt, 37.5 hours per week
  • Work hours mostly Monday-Friday approx. 9am-5pm with ability to occasionally work a schedule in response to the needs of the target population (occasional evening and/or Saturdays to meet class member needs)
