KHS reasonably expects to pay starting compensation for the full time position of Community Support Services Outreach Worker Bilingual I, in the range of $19.56 - $25.64 hourly. Our Mission.. Kern Health Systems is dedicated to improving the health status of our members through an integrated managed health care delivery system.Position Summary: This position reports to the Community Supports Services Supervisor. The position provides support to the CSS Program to ensure engagement, enrollment and follow up on members related to the CSS. This position will also play a central role in partnering with the CSS Community Based Organizations. Essential Duties and Responsibilities: Has a basic understanding of theories of change, motivational interviewing, and patient centered medical needs.Knowledge of the healthcare industry and familiarity with managed care plan designs and benefitsHelps member in navigating the health care system including access to health care, understand the health care delivery system, or engage in their own care.Promote a member’s health or addressing barriers to physical and mental health care, such as through providing information or instruction on health topics.Professional communication and ability to collect and share data with Community Based Organizations.Provide members with community resources.Outreach to members and tailor the conversation according to their readiness to enroll in CSSAddress member concerns with specialist referrals and appointments, transportation to medical services etc.Resolve CSS member concerns identified through Member ServicesCoordinate internal and external referrals to Community Based OrganizationsMake outbound calls using effective outreach techniques to promote member engagement to enroll in CSSAnswer inbound calls to determine member needs and promote member engagement into the CSSConduct outreach aimed at enrolling individuals who are eligible for the CSS program.member incentive programResearch other phone numbers and contact information if documented information is inaccurate.Use problem-solving and critical thinking skills to respond to member’s concerns. Have a thorough knowledge of KHS departmental resources and ability to coordinate members care with different departments.Assists management staff in completing member services related special projects.Utilizes computer for detailed data entry in multiple applications including Microsoft Office, JivaA willingness to work effectively in a teamAssist member with any inquiries regarding CSS services.Assist with member grievances as needed.Facilitates communication and coordinates services between providers and patients.Complete Qnxt assigned call tracking on a daily basis.Complete Jiva cross functional internal Health Services referrals.Provide staff coverage as neededRegular Predictable attendanceOthers duties as deemed necessary Employment Standards: Education: High School Diploma from an accredited school. ANDExperience One (1) or more years of recent experience in customer service-related field with a high degree of problem solving and decision making. Strong telephone and communication skills required. *Spanish/English Bilingual Certification required* Other: Required travel up to 20%. Possession of valid California Driver’s License and proof of valid State required auto liability insurance. Knowledge of: Use of Microsoft word, excel or other computer softwareUnderstanding the community services, community connectednessAbility and willingness to provide emotional support, encouragement, and motivation to members.Excellent customer service and telephone techniques and etiquette. Effective communicationDemonstrating exceptional teamwork skills and thrive in a collaborative work environment. **Bilingual Spanish/English (Tier I) certificate strongly preferred.
