As our Full-time Palliative Care nurse practitioner, you will be assigned to a skilled nursing facility (SNF) in Frederick, MD to treat geriatric patients for their Palliative Care and pain management needs. You will be responsible for Palliative Care throughout the SNF and maintain a strong relationship with the client facility, patients and families. Excellent clinical and interpersonal skills are required as well as the desire to be a part of a team that functions at the highest level. Your duties will include collaboration with administration, nursing, therapy, and families. The ideal candidate will always put the patient and facility first.
