Reports to : Coordinated Entry and Supportive Housing Manager

Job Type : Full Time

Salary : $20.50 hour

POSITION SUMMARY

Our Coordinated Entry and Supportive Housing (CE&SH) team partners with social service organizations to provide care to homeless and low-income individuals, identify long-term housing solutions, and support households in securing permanent housing.

The Homeless & Recovery Outreach Team will lead the outreach for CE&SH. The CE&SH Homeless & Recovery Outreach Team will also be responsible for documenting information and services into the Homeless Information Management System (HMIS) and tracking CE&SH performance measurements for the Homeless Crisis Response Policy to meet targets and benchmarks.

They will establish and maintain effective partnerships with other area landlords, and especially providers of mental health and substance use support.

DUTIES & RESPONSIBILITIES

  • Lead CE&SH in outreach, meeting clients where they are located, including, but not limited to treatment facilities, jail, encampments, or city and rural streets or other locations.
  • Meets with most critical individuals who may be homeless to determine their emergency and / or permanent housing needs, screens individuals for program eligibility, and coordinates referrals with the appropriate housing providers
  • Working closely with Kitsap County's Homeless Encampment Action Response Team (HEART) Coordinator to respond to encampments that the Coordinator has identified and requested for outreach and engagement activities.

Reporting any new encampments or significant developments with active encampments to the HEART Coordinator as soon as possible

  • Developing and following a regular weekly route of outreach and engagement with active encampments to build relationships with residents and monitor encampments
  • Responding as soon as possible to Kitsap County staff requests for outreach visits and then reporting back findings and results of the outreach visit (typically via email)
  • Providing regular periodic data reports on encampments, outreach, and engagement activities to Kitsap County (including information such as number of encampments visited, number of encampment visits, number of people engaged with, and number and type of resources offered, number of county outreach requests responded to, etc.).
  • Attend meetings with partner agencies as requested, representing CE&SH and KCR.
  • Develop and maintain a working knowledge of KCR and Community Partner programs.
  • Accept and engage in training as a Family Development specialist, stay current with FDC and all other required certifications as needed.
  • Identifies a household's primary housing barriers (employment, substance abuse, mental health, education, domestic violence, and health) and coordinates resource referrals for these services.
  • Maintains complete and accurate client files and quality data into the HMIS data system.
  • Assists the team in collecting, tracking and compiling client statistics; completes program reports.
  • Report to and accepts assignments from the Coordinated Entry and Supportive Housing Manager.
  • Accepts other job-related duties as assigned.

EDUCATION / EXPERIENCE

  • Requires High School Diploma or GED plus four years' experience or
  • AA Degree plus two years' direct client services experience.
  • Recognition and understanding of the socio-economic problems and needs of homeless and low-income families and individuals
  • Familiarity with available community resources
  • Must have valid Washington State Driver's License and current auto liability insurance
  • Must have reliable transportation to reach back and forth between various encampments and KCR facilities

ESSENTIAL PHYSICAL / MENTAL FUNCTIONS

  • Requires walking, reaching / stretching, intermittent sitting, twisting and bending.
  • Must be able to occasionally lift up to 20 pounds.
  • Works outside, subject to varying weather conditions.
  • Must be cognitively able to assess conditions in working with those in encampments and manage personal safety.
  • Must be able to successfully communicate both in writing and verbally with a wide variety of people with a broad range of backgrounds and circumstances, in a positive and non-judgmental manner.
  • Must have the cognitive ability to translate conversation and interactions into actionable steps designed to alleviate client needs
  • Must have the cognitive ability to manually and electronically record and track client-related data
  • Demonstrated ability to maintain complete and accurate information and records
  • Demonstrated emotional and cognitive ability to remain calm and provide appropriate advice and information with a trauma-informed framework.
