Description

• ALL APPLICANTS MUST HAVE Physical Therapist Professional license in Massachusetts.
• 1 Year Home Care experience required.
• Weekend rotations are once every 6 weeks. Ability to travel to cases as assigned.

Job Summary:

• Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes.
• Patients may range in age from birth to over 100 years.
• Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs.
• Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of MGH Home Care.

Job Type: Contract

Pay: $72.00 per hour

Schedule:

• Day shift
• Monday to Friday
• Weekends as needed
