MENTAL HEALTH COURT CLINICAL CASE MANAGER
Purpose of Job
This classification aims to assist the Mental Health Court Team with participants' progress to ensure participants comply with Mental Health Court program requirements. Duties include but are not limited to: determining mental health eligibility for all potential participants referred, completing an intake/initial case management plan for incoming participants, making service and level of care recommendations, linking, monitoring, and coordinating participants to identified services and supports, and collaborate with treatment providers to assure that participants are attending and making progress. The MHCCCM will create Case Management Plans for each participant for each Program Phase, identifying the requirements to which the participant will be held and the competencies they are expected to achieve in each phase of the program. The MHCCCM will maintain case notes in TNWITS and within internal standardized records documentation.
Essential Functions
The Mental Health Court Clinical Case Manager is responsible for the following essential functions:
- Conduct initial screening of potential participants. Conduct mental health screening, including a clinical Summary based on TNRAS (Training and Certification provided by TDMHSAS Grant), CAGE-AIDE, Modified Mini Screen (MMS), Adverse Childhood Experience Questionnaire (ACE), and Columbia Suicide Severity Rating Scale (C-SSRS), and prepare solution-focused Case Management Plans.
- Link program participants to community mental health services for treatment and care. Additionally, link participants with a continuum of care, including primary health services and public health services.
- Complete detailed participant progress reports/notes. Continual monitoring and case management plan modification for phase progression.
- Teach, role model, and coach participants on how to use resources and navigate or exercise skills needed for managing mental health symptoms and/or substance use or medication (if applicable) and hold participants accountable for their actions.
- Teach, role model, and coach effective communication with doctors, therapists, and attributes needed to attain and maintain employment and housing.
- Develop relationships and linkages with mental health agencies, treatment providers, harm reduction service providers, primary healthcare providers, specialty healthcare providers, hospital partners, and other community agencies.
Additional Job Functions
- Performs other related duties as required.
- Participate and lead continuous improvement initiatives.
- Participate in research and program evaluation.
- Provide statistics as requested by administration.
- Facilitate with Treatment Service Provider Partners onboarding and acceptance.
- Assistance with housing stabilization, if applicable.
- Meets with assigned Phase 1-5 participants as outlined in the KCMHC program.
- Monitor's participant's activities, work status, and living arrangements.
- Assists with the transportation coordination and/or transportation when applicable for Mental Health Court participants.
- Attends all Mental Health Court Staffing Sessions.
- Serves as liaison between the court, law enforcement agencies, treatment provider partners, community correction/probation, and community.
- Maintain continuing education units as required by the state.
- Should attend the Annual State and/or National Drug Court Conference.
Minimum Training & Qualifications
- Master's or Doctorate Degree in Social Work.
- Social work clinical experience, minimum of five years.
- Make evidence-based decisions.
- Ability to focus and demonstrate excellent attention to detail.
- Must be proficient in Windows OS and Microsoft Office.
- Must have and maintain a valid Tennessee Driver's License.

- May require occasional travel and on-call duties.
