Job Title: Registered Nurse (Case Manager)
Location: Boston, MA, 02114
Shift: Days | 07:30 - 17:00
Schedule: 10 hours x 4 Days per week
Contract Duration: 13 weeks, possible extension
Job Description:
We are seeking a dedicated and compassionate Registered Nurse (Case Manager) to join our team in Boston, MA. This position offers a day shift schedule with 10-hour shifts, four days a week. The initial contract is for 13 weeks, with the possibility of extension based on performance and staffing needs.
Responsibilities:
- Coordinate and manage patient care plans from admission to discharge, ensuring continuity and quality of care.
- Assess patient needs, develop care plans, and monitor patient progress.
- Collaborate with healthcare professionals, patients, and families to provide optimal patient outcomes.
- Educate patients and their families about medical conditions, treatment plans, and available resources.
- Ensure compliance with healthcare regulations and standards.
- Document patient interactions and updates accurately in electronic health records.
- Participate in interdisciplinary team meetings and case conferences.
Qualifications:
- Current and valid Registered Nurse (RN) license in the state of Massachusetts.
- Proven experience as a Case Manager or similar role in a healthcare setting.
- Strong clinical skills and knowledge of healthcare regulations and standards.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
