I. Position Function Summary

Under the direction of the Patient Programs, Director, the HCH Clinical Manager is dual role responsible for planning, organizing, and directing the day-to-day activities of La Familia Medical Center’s (LFMC) Health Care for the Homeless program (HCH) and medical clinic; and responding to the needs of patients by developing, implementing, and managing programs that address the problems and needs associated with the condition of homelessness.
Additionally, this role will be 33% patient facing for direct patient appointments, with additional support given to the other providers of the HCH clinic for consultation management of complex behavioral health issues. Under the direction of the CMO, the Certified Psychiatric Nurse Practitioner provides psychiatric and preventive care to patients with acute and chronic mental illnesses in an assigned clinic site(s), in accordance with federal, state, and other administrative requirements.
II. Essential Responsibilities
  • Ensures appropriate health care, case management, and referral services, as well as the general well- being of LFMC/HCH homeless patients by developing, implementing, and continued evaluation of the HCH program with special attention to the problems associated with homelessness; by ensuring that:
  • Ensures that LFMC/HCH is a community leader on homeless issues and health care by working with homeless services providers, collaborative partners, community services, health care providers, and other community entities to facilitate clinical affiliations and by overseeing outreach to the community, training of volunteers, and coordination of referrals from LFMC/HCH providers to community supports and services;
  • Contributes toward operational success of LFMC by developing, monitoring, and complying with the budget for the HCH, participating in quality improvement initiatives, and continually evaluating performance indicators delineated in the business and clinical plans for LFMC;
  • Directs and supervises reporting personnel in such a manner as to promote a safe working environment, safe operation of LFMC, productive efficiency of employees, and managing the Department in keeping with state and federal laws and LFMC’s policies, guidelines, and EEO/affirmative action policies;
  • As a member of the LFMC management team, participates in strategic and operational planning; translates the goals, strategy and programs of LFMC into specific and meaningful work assignments for the department, delegating assignments and responsibility to direct report staff in order to accomplish program goals and objectives;
  • Coordinates with health education program to provide services, identify target health issues and geographic areas to be served and prioritize health education goals and services for identified communities as well as individual patients;
  • Coordinates with Chief Clinical Officer to ensure safety and functionality of clinical systems by overseeing maintenance and repair of all medical and nursing equipment at HCH;
  • Oversees employee duties and work schedules, including staff leave and department specific training needs. Responds promptly to performance issues. When applicable, assesses and documents staff competencies and addresses deficiencies through appropriate training.
  • Enlists assistance of departments and staff with subject matter expertise as appropriate.
  • Appropriately communicates organization information through department meetings, one-on-one meetings, appropriate email, and regular interpersonal communication. Informs employees of new policies and procedure changes. Ensures that employees comply with policies, laws and regulations.
  • Maintains professional and technical knowledge by participating and completing LFMC training and external educational opportunities. Stays informed of policy, compliance and legal expectations affecting department.
  • Maintains Certified Psychiatric Nurse Practitioner practice at LFMC as authorized by the Psychiatric NP license and in accordance with the certified psychiatric nurse practitioner job description. Works in area of his/her specialized field in delivery of care to LFMC patients; demonstrates differential diagnosis capabilities, makes psychiatric/behavioral health judgment and decisions based on history and physical exam observations on patients and formulates treatment plan in patient psychiatrist records. Evaluates outcomes and modifies treatment plan accordingly;
  • Provides patient care by evaluating and diagnosing patients, and carrying out psychiatric treatment, behavioral health therapy, and follow-up; referring patients after consultation and treatment back to primary care providers, and behavioral health clinicians when stable; educating patients in behavioral health and self-care; documenting patient-doctor transactions for proper record keeping and billing;
  • Assists and receives direction in the provision of patient care by consulting with LFMC physicians psychiatrists, and behavioral health staff on the following situations, as needed:
  • when required by protocol;
  • when faced with a situation which may require hospital admission;
  • when incumbent is medically or psychiatrically uncertain in a particular clinical situation;
  • when the clinical situation is beyond the scope of practice of the incumbent;
  • Conducts psychiatric evaluations for all age groups to include health histories, labs, and other tests as appropriate with a focus on prevention and psychiatric intervention as indicated;
  • Diagnoses and treats behavioral and psychiatric illnesses, short term as well as chronic and severe;
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed psychiatric conditions;
  • Prescribes medications, orders refills and maintains all required pharmacy logs;
  • Orders laboratory tests, interprets results and communicates results to patients;
  • Consults with physicians and other medical specialists and refers patients to an appropriate provider;
  • Performs specialized psychiatric assessments and behavioral health screenings as needed;
  • Charts and documents all patient interactions appropriately in the electronic medical record, responds to messages and requests from nursing staff and patients, conducts chart audits for clinic, completes charting in required timeframe and conforms to meaningful use and other guidelines as required;
  • Participates in education and training of nurses and behavioral health staff to improve their clinical skills;
  • Works with other providers, Behavioral Health Director, and counseling staff to provide a team approach to psychiatric care;
  • Provides support in the overall LFMC operation by acknowledging, adhering to, and implementing overall organizational and medical policies and operating procedures of LFMC;
  • Provides counseling to individuals, parents and/or children, focusing on preventive mental health care; educates patients by disseminating patient information and literature relating to health and mental health education including but not limited to family therapy, individual and specialized forms of treatment and health promotion/disease prevention for all age categories;
  • Maintains compliance of clinic with state licensure by enforcing Quality Assurance protocols in the clinical, pharmacy, laboratory, medical supply, and patient care areas of LFMC for quality patient care;
  • Maintains and updates professional knowledge and skills, and maintains psychiatric and clinical proficiency by attending continuing education or seminars related to job responsibilities, including Age Specific Training (care giving, treatment, and assessments specific to the age of populations served).
I. Education, Experience And Skills/Training
  • This position requires that the Certified Psychiatric Nurse Practitioner be a graduate of an accredited Certified Psychiatric Nurse Practitioner program;.
  • Preferred experience in a community health/behavioral health center,and working within a managed care environment.
  • Documentation of education, experience and licensure is required and must be maintained; Certified Psychiatric Nurse Practitioner must be insurable under the policies and requirements of LFMC;
  • Current licensure for the state of New Mexico is required;
  • Certified in specified area of medical specialty by American Academy of Nurse Practitioners or the American Nurses Credentialing Center;
  • Federal DEA Certification;
  • NM State pharmacy license;
  • ACLS or CPR and First Aid certification;
  • Must be credentialed per LFMC procedures.
  • Four-year degree from an accredited institution required. Background and education in Public Health preferred, with a minimum of three years supervisory experience. In lieu of a degree, must possess sufficient prior work experience in progressively more responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.
