Job Description

  • Receives and follows schedule/instructions for resident care as assigned from the director of nursing and in accordance with established policies, procedures, and regulatory guidelines.
  • Performs focused interview to identify specific resident needs.
  • Assesses signs and symptoms indicating physiologic and psycho-social changes in the resident’s condition and notifies supervisor and documents in PCC.
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential diagnoses, coordinates care with the interdisciplinary team (IDT) and documents in resident electronic chart (Point Click Care/PCC).
  • Works with the nursing team assigned as primarily responsible for the resident. 
  • Communicates with the physician as needed and ensures communication of resident care needs is electronically documented.
  • Complies with PHI and HIPAA policies regarding any disclosure of resident status.
  • Plans for outcomes of care for those residents assigned.
  • Performs interventions according to identified priorities, plan of care, and the community policies and adheres to emergency, safety, and infection control procedures of the community.
  • Promotes and participates in activities to facilitate effective interpersonal communications.
  • Reports changes in the resident’s respiratory status to the Directors of Nursing and Respiratory Care.
  • Develops and maintains a good working rapport with inter-department personnel.

Minimum Qualifications:

  • Freedom from illegal use of drugs.
  • Must pass State Abuse Registry and Stellar criminal background screens.
  • Demonstrates effective time management skills.
  • Must possess, as a minimum, an associate degree from an accredited school.
  • Minimum of one (1) year respiratory experience in a skilled nursing and/or acute care setting.
  • Graduate of an accredited Respiratory Therapy program.
  • BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer.
  • Registered or Certified Respiratory Therapist.
  • Licensed Nurse trained in respiratory therapy.
  • Knowledgeable user of Point Click Care (PCC) preferred.
