  • Location: Aurora, CO (Hybrid)
  • Employment Duration: 6 months
  • Pay Rate: $23.00/hour
  • Work Schedule: Offers remote flexibility with two in-office day per week and 3-10 client visits weekly

About the Role: This position offers a blend of remote work, in-office collaboration, and engaging community interactions. As a Nonprofit Case Manager, you'll manage individuals receiving waivers and State General Fund services. Your responsibilities will include maintaining essential documentation, coordinating interdisciplinary team (IDT) meetings, and ensuring the effective implementation of Service Plans (SP).

Key Responsibilities:

  • Serve as the subject matter expert for applicable programs and services, acting as a resource and point of contact for complex cases.
  • Provide care coordination services and interventions, including referrals, education, negotiation, and mediation with individuals and external providers/agencies.
  • Accurately complete case documentation in appropriate tracking systems and databases, prioritizing workload to meet all required timelines.
  • Complete required documentation related to monitoring activities and other targeted case management activities; evaluate the need and effectiveness of requested and delivered services.
  • Ensure the accuracy and timely entry and distribution of required documentation to all necessary team members, including providers, individuals, and family members.
  • Maintain knowledge of state and federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver programs, state-funded programs, and other community resources.
  • Uphold a professional and ethical manner in all interactions, meeting performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries.
  • Manage a caseload of individuals receiving waiver and State General Fund services, maintaining required quarterly and annual documentation.
  • Complete required documentation related to monitoring activities, ensuring the effectiveness of services delivered as authorized by the service plan and that identified outcomes are met; advocate for individual service needs.
  • Facilitate interdisciplinary team (IDT) meetings to coordinate services to meet individuals' needs; ensure appropriate interaction and communication; implement IDT meetings as needed to modify services.
  • Conduct assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviews, observations, and the utilization of standardized tools.
  • Schedule, facilitate, document, and complete follow-up for all required meetings; disseminate updates and obstacles to necessary team members.
  • Manage caseloads to ensure support is provided in alignment with rules and regulations.
  • Complete coverage tasks, including required documentation related to screening and referral, eligibility determination, assessments, support plan development, monitoring activities, and other assigned duties.
  • Ensure the effectiveness of services delivered, advocate for individual service needs, and provide support with coverage management, research, and assignments.
  • Work with the Program Manager to determine needs among coverage, assist in completing hand-offs, and support department needs as assigned.

Requirements:

  • Bachelor’s Degree - OR - five (5) years of experience in the field of Long-Term Services and Supports (LTSS), including Developmental Disabilities, or a combination of education and relevant experience
  • Valid motor vehicle license, proof of insurance, and a reliable vehicle
  • Clean driving record (no suspensions and no more than two tickets in the past three years; driving requirements may be waived with reliable transportation)

Preferred Qualifications:

  • One (1) year of experience in developmental disabilities/long-term services and supports
