Summary

The Targeted Case Manager acts as a member of a multidisciplinary team to assess and coordinate patients’ care among multiple service providers during inpatient and outpatient treatment.

Responsibilities

• Assess patient biopsychosocial stressors (i.e. housing, financial, legal, medical, employment, etc.) and developing a strategy to meet these needs during and after treatment episode

• Facilitate multiple care aspects (case coordination, information sharing, aftercare planning, court related issues)

• Maintain a good working relationship with support networks, government resources, community resources, employers, and referral sources.

• Complete of all necessary documentation related to FMLA and Short-Term Disability

• Maintain patient records by reviewing and completing case notes

• Prepare patients’ discharge by reviewing and coordinating community services necessary to maintain sobriety outside of structured setting

• Participate in weekly multidisciplinary treatment team as part of ongoing assessment of patient needs and plan for discharge

• Be involved in census management including coordinating discharge schedule and communicating to treatment team and administrative team

• Must continuously maintain license via Continuing Education as required

Qualifications

• Bachelor’s Degree or higher in a behavioral science preferred or a CADC with a Bachelor’s Degree

• Previous Targeted Case Management experience or supervision of the service since 2014

• Valid, unrestricted driver’s license

• Clean driving record for at least 3 years

• 1 year of experience working directly with adults or adolescents in human service setting Issue

• Commit to a drug-free lifestyle and responsible alcohol use

• Passion or interest in substance abuse issues strongly preferred
