Laurel LifeJob DescriptionJob Title: Mental Health Professional FLSA: Exempt Reports To: Program Supervisor Department: Family Based Mental Health Services SummaryThe Mental Health Professional (MHP) is one of two therapists that provide family based treatment within the home and community. The purpose of Family Based Mental Health Services (FBMHS) at Laurel Life is designed to prevent inpatient hospitalization, as well as any other out-of-home placement, and aid in the reunification of families when children have been placed outside the home.
Services will be delivered in the home, school and community by a team of mental health staff who are trained in Eco-systemic Structural Family Therapy (ESFT) Logic Model principles. Delivery of service includes 5-10 hours per week of intensive therapy. Treatment sessions includes individual, parenting, and group therapy with all family members to promote effective communication and skill transference. The FBMHS team goal is to intentionally model strategies to work toward family empowerment in repairing ruptured relationships.

  • 24/7/365 Wellness Support for all families identified in care.
  • Successful discharge planning, from the initial date of service, to a lesser level of care
  • Case management of connecting families to all necessary community supports during the provision of services.
In addition to the details and requirements listed below, Folium, Inc. expects the employee to be committed to incorporating principles of trauma informed care in all areas of service delivery. Commitment to these principles in trauma treatment will be required through attendance in trainings and use of identified evidence-based practices.? Trauma informed care includes cultural sensitivity and emphasizes a safe and respectful environment for all clients and staff.??
Duties And Responsibilities
  • Complete individual and family assessments outlining specific strengths and needs with the identified member and other members of the family systems.
  • Develop the preliminary treatment plan within 5 days of the initial intake. A more in-depth treatment plan which is a cooperative effort with all involved CASSP agencies must be completed within 30 days of contact. The Mental Health Professional (MHP) is then responsible to prepare, facilitate, and complete treatment plan reviews every 30 days.
  • Provide a minimum of 30 billable hours per week of direct service in the form of an individual, parents, family and collateral sessions. Hours of need range from 4 to 10 hours per week, per family. Caseloads will range from 6-8 families with 8 being the maximum per state regulation.
  • A team-delivered therapeutic service will be provided to our members utilizing a 60(team)/40(individual) treatment ratio. All services provided are to be reflective of the treatment plan goals or indicated as a need on the treatment plan.
  • MHP's will be included in a rotation of on-call provision to all FBMH members within a program. On-call services include 24-hour, 7 days per week availability for crisis intervention support, including phone response within 15 minutes of the initial phone call. Crisis calls may require an on-site visit, which will occur within 1 hour of the initial phone call. The Program Supervisor will be notified of all calls that are made to the on-call number. Supervisors are also available to provide assistance if necessary.
  • Develop cooperative relationships with community and contractual agencies so as to facilitate treatment services provided to members and their families.
  • Assess need for Family Support Services funds used to meet social determinants of health.
  • Collaborate with other treatment team members to enhance community supports and outcomes.
  • Complete required documentation following each completed session, assessments, treatment plan, and discharge.
  • Certification in Safe Crisis Management, including Emergency Safety Physical Interventions (ESPI). ESPI techniques are only used in compliance with Folium policy and procedure and within guidelines established by third-party experts in the proper and legal use of ESPI.
  • Maintain an active valid driver's license issued by state in which they reside and must immediately notify their supervisor or Human Resources if their license is suspended or revoked for any reason.
  • Complete all clinical and didactic trainings through Philadelphia Child and Family Therapy Training Center, Inc. (PCFTTC) as scheduled.
  • Successfully complete all necessary requirements for preparation for FBMHS State Certification.
  • Complete all assigned Folium mandatory trainings via SafeSchools portal.
  • May perform other duties, activities or special projects as requested by the Family Based Program Supervisor and Director.
Knowledge And SkillsA Mental Health Professional demonstrate excellent organizational and interpersonal skills, knowledge of clinical issues, treatment strategies and the ability to develop meaningful relationships with members and their families. MHPs must be able to remain calm, identify underlying causes of problem situations, resolve conflict and de-escalate potentially explosive situations with the utilization of the Eco-systemic Structural Family Therapy Model.
Education And Experience Requirement
  • A graduate degree in counseling, psychology, social work, nursing, education, rehabilitation or any other related field of human services plus 2 years of experience in a Child and Adolescent Service System Program (CASSP) agency or,
  • Be a licensed registered nurse (RN) with 5 years of experience including 2 years of experience in a CASSP agency, plus be a Certified Family Based Mental Health Worker by the Department of Human Services (DHS) Office of Mental Health and Substance Abuse Services (OMHSAS).
  • A bachelor's degree in psychology, sociology, social work, nursing, rehabilitation, education, pre-med, theology, anthropology or any other related degree in the field of human services plus be a Certified Family Based Mental Health Worker by the Department of Human Services (DHS) Office of Mental Health and Substance Abuse Services (OMHSAS).
Working ConditionsThis is an intensive in-home clinical position which is primarily performed in consumer homes and the community. Due to the nature of the services performed, the employee in this capacity will be required to visit children/adolescents and their families in their homes and community settings on a weekly basis. This is a direct service position requiring daily travel.
