We are seeking a dedicated Case Manager to join our team. The Case Manager is a pivotal point in care coordination for our students. The Case Manager is skilled in working with families, referring agencies, and the student as it relates to their treatment and transition plans, as well as working closely with our clinical and residential departments to be certain critical information, case notes, and treatment goals are communicated. The Case Manager will be responsible for providing support and guidance to individuals in need, coordinating services, and advocating for clients to ensure their needs are met effectively.

The ideal candidate has strong communication and relationship building skills. They thrive in an environment where information moves fast and changes sporadically. This person is well written, spoken, and thoughtful in their approach as they work with a variety of people internally and externally.
