Position Summary? 

The Outreach Manager is responsible for supervising LCHC’s Outreach team and setting quality improvement and marketing goals for the team. This includes overseeing the team’s role in educating the public, community partners in care, and referral sources about the LCHC PACE program; organizing community outreach and promotional activities; and guiding interested elders into the intake process for PACE. As leader of the Outreach team, the Outreach Manager identifies and cultivates new relationship opportunities that will support the organization’s goals for enrollment growth and evaluates results. The Manager serves as the primary sales representative to individuals and organizations who serve the frail elderly as an essential part of this position. The Outreach Manager bridges the gap between our clinic’s quality goals and the targeted calls made by the Outreach team members.

• Clear and concise written and verbal communication skills for communicating professionally with patients and coworkers. 

• Ability to prioritize and perform a variety of tasks at once. 

• Flexibility to adapt to changing or stressful conditions, including changes to work schedule. 

• Ability to use an interpreter (or Spanish-speaking technician) when necessary to communicate with patients. Strong background in patient counseling.? 

• Strong interpersonal skills, including an ability to work well with the variety of ages, cultures, and backgrounds represented among LCHC staff, patients, and caregivers.?? 

Required Knowledge, Experience or Licensure/ Registration *? 

• One year of experience with a frail or elderly population is required. 

• Must submit to and pass a pre-employment screening and criminal background check. 

• Must have a current Driver’s License and maintain current automobile insurance. 

• Knowledge: Spanish is helpful 

• OIG background check performed prior to employment?
