Lawrence Hall is Hiring an Intake and Assessment Supervisor Near Chicago, IL

Essential FunctionsReports to the Vice President of Foster Care Services. This position depicts a dual role, which includes facilitating intake as well as permanency requirements for exiting the program.
Oversees all referral, intake and placement functions for the Foster Care Programs including HMR(home of relative), Spec (Specialized Foster Care), and AFC (Adolescent Foster Care).
The Supervisor manages relationships with referral sources and placement options, ensuring efficient foster care homes for youth entering care. This role manages exits through permanency planning. The Supervisor independently conducts adoption/guardianship investigations, processes subsidy documents (including interviews and approvals), and liaises with foster parent attorneys - all in compliance with federal and state regulations.
Requires knowledge of trauma exposure, knowledge of the 5 S's of trauma: Safety, Self-Regulation/Soothing, Supportive, Strengths, Self-Care, and an understanding of and recognition of the signs of traumatic stress. Must be able to effectively use both verbal and physical intervention skills with both youth and adults. Physical intervention skills require the ability to kneel and return to standing without assistance. Coordinator manages a high volume of correspondence, and keeps accurate and detailed records. Position requires exceptional organization and efficiency. Use of effective verbal and written communication with staff, referring agencies, internal and external members of Lawrence Hall, youth and families. Maintains flexibility to adjust work schedule according to Agency and programmatic needs.
Job Responsibilities
  • Reviews and distributes written referral packet, supporting documents for permanency (psychological, medical, progress reports, case reviews, socials, etc.) to all pertinent staff. Attempts to clarify or obtain all vital information through the referring agency.
  • Schedules interviews and appointments, and coordinates with unit and clinical staff regarding prospective and pending client referrals.
  • Facilitates weekly intake/permanency meeting with Foster Care staff to foster communication and provide seamless transition for youth entering and exiting the foster care program.
  • Maintains records on intake and discharge activity; writes summary reports and prepares statistical data reports as needed.
  • Actively involved in planning and delivery of training related to own area of expertise and needs of program.
  • Coordinates with case work staff to prepare necessary paperwork for subsidy completion (e.g. subsidy documents, investigatory reports, etc.).
  • The Supervisor ensures exceptional customer service through prompt responses to inquiries via phone and email.
  • Other duties as assigned.
Qualifications
  • Master's Degree in human services field, preferably in social work, with equivalent combination of education and experience
  • Minimum of three years of experience in the child welfare field. Previous intake and placement experience preferred
  • Must have the Child Welfare Employee Licensure
  • Must pass the Foster Care Licensing standards (Rule 402) within three (3) months of employment
  • Proficiency using computer programs, including Microsoft Outlook, Microsoft Word, and Microsoft Excel
  • Exceptional written and verbal communication skills
  • Valid driver's license, state required auto insurance and clean drivers record. Personal vehicle is required.
  • No history of child abuse or neglect
