JOB SUMMARY The Population Health Manager oversees care management activities by guiding the development, implementation, and monitoring of population health programs and quality improvement activities. The Population Health Manager oversees the Primary Care Health Home (PCHH) and Care Management team to help coordinate population health-related services with the goal of achieving positive health outcomes, program, and organizational goals. Job responsibilities include but are not limited to managing the Patient Centered Health Home and related areas and programs within the population health division daily operations, monitoring and measuring productivity, providing training to staff, resolving staff issues, and identifying areas of improvement. The Population Health Manager will work to lead and develop programs that align with government and payer requirements, including compliance with the National Committee for Quality Assurance PCMH Recognition standards.
