Summary
The Case Manager has clinical expertise with knowledge of medically appropriate criteria, and applies principles of utilization and quality management, discharge planning and the management of clinical/financial resources as a facilitator and active participant of the multidisciplinary patient care team. The Case Manager predicts, facilitates and evaluates the transition of patients across the care continuum and links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning.
Requirements
Education:
Associates Degree in Nursing with ongoing efforts to obtain Bachelors Degree.
Experience:
Minimum of 3 years clinical experience required. Previous experience in discharge planning, utilization management, knowledge of community resources, assessment skills and clinical and social factors that affect patients functional status at discharge preferred.
License:
Currently licensed as an RN in the State of Florida.
Certifications:
Case Management certification in: ACM, CCM, NACCM, or CMCN required within 5 yrs. employment.
Other:
Communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task and prioritize responsibilities.
