Leeza’s Care Connection (LCC) has been helping family caregivers since 2002. LCC takes pride in assisting caregivers in answering the question, “Now what?” when faced with a life-changing diagnosis of a loved one. With no shame or blame, LCC meets caregivers where they are and helps them craft the journey forward. We help families cope with a health crisis, care for themselves and their diagnosed loved ones, and create positive new realities for everyone through our network of connections.

Job Brief:

Under the direction of the Chief Program Officer, the Director of Community Care directs, manages, and oversees programming and special events while supporting the operations of Leeza’s Care Connection in Columbia, SC. The Director will coordinate with other stakeholders and outside agencies to assist with community needs and deliver innovative, consistent, high-quality care, community engagement, and resource management. A successful Director of Community Care can provide clinical leadership for all services and programs for the caregiver community.

Essential Job Responsibilities:

• Initiate and set program goals according to the organization’s strategic objectives.
• Manage programs from start to completion involving volunteer and participant outreach, tracking, marketing, and participant follow-up.
• Develop and create new programs to meet the needs of our community.
• Caregiver consultations offering support and resources
• Develop high-quality caring and trusting relationships with caregivers.
• Devise evaluation strategies to monitor performance and determine the need for improvements.
• Maintain accurate records and data on CRM program activities, demographics, and outcomes.
• Supervise all program staff and volunteers involved.
• Discover ways to enhance the efficiency and productivity of procedures and people.
• Initiate community outreach plan, including following up with leads and participating in marketing events and activities that correlate with the overall marketing plan.
• Manage special events, projects, and fundraising.
• Keep the Chief Program Director informed with detailed and accurate reports and updates.

Requirements:

• A Master’s degree in a related field preferred and a related licensure like LMSW.
• Compassionate and Passionate
• Knowledge of Dementia, Alzheimer’s, Family Caregivers, and aging services
• Proven experience in a social services role or counseling role
• Management or leadership experience
• Knowledge of data analysis and reporting
• Organizational, problem-solving, and leadership skills
• Computer skills, including skills in Microsoft Office Suite, Excel, and Google Drive, and comfort with learning/using database and software platforms
• Excellent communication skills

Location: Columbia, SC

Hours: Full-time

Salary Range: $52,000-$62,000
