Legacy Community Health is hiring immediately for Pharmacy Advocate through our partnership with the AmeriCorps Program. AmeriCorps is a Federal Program that connects Americans with Community Services in Health, Public Safety, etc.
The session begins September 3, 2024. Must commit to completing the 1700-hour requirement by August 31, 2025. The bi-weekly (every two weeks) stipend is $700, less applicable taxes.

About the Position
Helps reduce patient barriers to accessing pharmacy services by increasing patient knowledge of all available pharmacy services, addressing social determinants of health, and connecting patients to resources. The Pharmacy Advocate will play a vital role in increasing access to affordable medications, promoting health literacy, and improving health outcomes for individuals and families in need.
Key Responsibilities

  • Utilize health information technology tools, such as electronic health records (EHRs) and pharmacy management systems, to streamline pharmacy workflows, enhance communication between healthcare providers, and improve medication safety and quality of care.
  • Provide technical support to patients and healthcare providers using pharmacy applications to access medication counseling, refill requests, flavoring, and medication delivery services.
  • Educate patients on the proper use of self-monitoring devices, such as blood pressure monitors, and glucometers, to empower them to manage their health conditions effectively at home. Provide step-by-step instructions on device setup, calibration, and operation, ensuring patients understand how to obtain accurate readings and interpret results.
  • Collaborate with pharmacy staff and healthcare providers to identify patients at risk of non-adherence and prioritize outreach efforts based on patient needs and medication urgency. Provide personalized support and education to patients regarding the importance of medication adherence, potential barriers to adherence, and available resources for medication assistance or counseling.
  • Support pharmacy's preventative health initiatives by educating patients on recommended immunizations, smoking cessation, weight loss and other initiatives.
  • Identify and/or develop handouts, presentations, resources, models, and other teaching tools and materials to effectively convey pharmacy information.
  • Increase patient awareness and knowledge of various medication and insurance assistance programs and services available regardless of patient payor status. Help individuals navigate the application process, gather required documentation, and follow up on eligibility status.
  • Organize brainstorming sessions to generate creative ideas for optimizing inventory processes and overcoming logistical challenges
  • Participate in quality improvement initiatives aimed at enhancing the delivery of pharmacy services and improving patient outcomes. Identify areas for improvement through data analysis, stakeholder feedback, and observation of pharmacy workflows.
  • Participate in a variety of educational experiences such as patient case presentations, journal clubs, pharmacist shadowing and lectures.
  • Foster relationships with local healthcare providers, agencies, and community leaders to leverage resources and maximize impact. Collaborate on joint initiatives, share best practices, and identify opportunities for collaboration to enhance the delivery of pharmacy services.
  • Perform other duties as assigned.
In addition to the activities above, all Legacy Community Health Corps Members will:
  • Represent Legacy's AmeriCorps Program by wearing the AmeriCorps logo daily
  • In the event of a local, state, or federal disaster declaration, AmeriCorps members may be asked on a volunteer basis or be required by OneStar to activate to assist in disaster response and recovery activities within their communities. This may involve virtual service opportunities or in-person service opportunities. This may include service opportunities outside of the member's standard service hours or in place of their standard service duties. In the event of a large-scale, high needs local, state, or federal disaster, AmeriCorps members may voluntarily or be required by OneStar to deploy to an area outside of their service area to assist in disaster response and recovery activities.
Nonessential Functions: Legacy Community Health Corps Members may have opportunities to engage in the following activities:
  • Work with community partners to identify service opportunities in alignment with the program design.
  • Members may be asked to perform other position-related instructions within the scope of their normal service activities as requested by their site supervisor and/or AmeriCorps program staff in alignment with the grant and not unallowable or prohibited activities and subject to reasonable accommodation.
  • Provide translation and interpretation services as appropriate.

Requirements
  • High School diploma or GED is required.
  • No previous experience is required.
  • Bilingual / Spanish fluency is preferred.
  • U.S. Citizen or a Permanent Resident
