LEGACY HEALTH is Hiring a Remote Resource Specialist-hybrid-remote

Overview

This is a hybrid-remote position - incumbents, who reside in Oregon or Washington only, may work 2-3 days a week at an assigned Legacy Health location, and other days may work remotely at home, on the road or in a satellite location for the remainder of their workweek.

All new hires are required to come to a designated Legacy Health office location in Portland, Oregon prior to their start date for a new hire health assessment and to complete new hire paperwork. This position may require initial training and orientation to be site-based, before transitioning to the hybrid schedule.

They do this by evaluating the psychosocial issues of patients to ensure transition planning, crisis management, and planning/coordination of both internal and external services. If you want to grow as an empathetic advocate for the needs of vulnerable populations, you may be perfect for our Legacy team.

The Resource Specialist will provide Care Management services to patients who have been identified by the Population Health team, or patients who self-identify. Keys to success include customer experience; motivational interviewing; facilitating access to community resources; coordination with community organizations on behalf of patient; crisis management; access to medications and insurance; transition planning; psychosocial assistance. This position works as part of multidisciplinary team which may include RN Care Managers, Health Coaches, Clinical Pharmacists, clinic providers and staff, and other Care Management staff. 

Qualifications

Education/Experience:

Bachelor's Degree or any combination of equivalent education and case management experience.

Experience in community-based case management

Experience in assessment and care plan development.

Skills:

Strong assessment skills and ability to assess urgency of need, and triage for services

Knowledge of community resources available to patients, caregivers and families

Knowledge of the community social service delivery system.

Ability to integrate knowledge of the normal aging process, changes related to chronic illness, and needs of patients with chronic illness.

Excellent organization, oral, and written communication skills for effective interaction with patients, patient’s families, physicians and staff

Strong advocacy skills

Computer ability and knowledge for entering and retrieving patient data, preparing letters and summary information

Ability to foster communication and build relationships among Care Management team members, providers and clinic staff

Excellent time management skills

Ability to read and interpret medical records
