Responsibilities:
- Conduct comprehensive physical assessments and obtain detailed patient histories.
- Diagnose and treat acute and chronic illnesses, injuries, and infections.
- Develop, implement, and evaluate individualized care plans.
- Prescribe medications and other treatments as appropriate.
- Order and interpret diagnostic tests, such as lab work and imaging studies.
- Provide patient and family education on health maintenance and disease prevention.
- Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care.
- Maintain accurate and up-to-date patient records.
- Participate in quality improvement initiatives and evidence-based practice.
- Adhere to all relevant legal, regulatory, and institutional policies and procedures.
- Master's degree in Nursing (MSN) from an accredited program.
- Current licensure as an Advanced Practice Registered Nurse (APRN) in Kentucky
- National certification as a Nurse Practitioner (NP) or Clinical Nurse Specialist (CNS) in a relevant specialty.
- Proven experience as an APRN, preferably in Family Practice
- Strong clinical skills and knowledge of evidence-based practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to providing high-quality, patient-centered care.
