The Home Health Care Coordinator is responsible for managing and overseeing the delivery of home health services to patients. This role involves coordinating care plans, ensuring compliance with healthcare regulations, and facilitating communication between patients, families, and healthcare providers to deliver high-quality and personalized care.

Key Responsibilities:

• Care Coordination:
• Develop, implement, and monitor individualized care plans in collaboration with patients, families, and healthcare providers.
• Coordinate patient care services, including scheduling visits, managing patient records, and ensuring continuity of care.
• Conduct initial patient assessments to determine care needs and coordinate with appropriate healthcare professionals.
• Communication and Liaison:
• Serve as the primary point of contact for patients and families, addressing concerns and providing support.
• Facilitate effective communication between patients, caregivers, and multidisciplinary teams to ensure cohesive care delivery.
• Educate patients and families about care plans, treatment options, and available resources.
• Compliance and Quality Assurance:
• Ensure adherence to healthcare regulations, policies, and procedures, including patient confidentiality and safety standards.
• Monitor and evaluate the quality of care provided by home health aides, nurses, and other healthcare professionals.
• Prepare and maintain accurate documentation and reports related to patient care and service delivery.
• Administrative Duties:
• Manage patient intake and discharge processes, including coordinating referrals and ensuring all necessary paperwork is completed.
• Oversee scheduling of staff and patient visits to optimize resource utilization and efficiency.
• Assist with billing and insurance processes, including verifying patient eligibility and handling claims.
• Team Collaboration:
• Collaborate with healthcare providers, including physicians, nurses, and therapists, to ensure comprehensive patient care.
• Participate in team meetings, case reviews, and training sessions to enhance care delivery and professional development.

Qualifications:

• Certification or licensure as a registered nurse (RN) or other relevant healthcare professional preferred.
• Minimum of 2 years of experience in home health care, case management, or a related field.
• Strong knowledge of home health care practices, healthcare regulations, and patient care coordination.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work independently and collaboratively in a fast-paced environment.

Working Conditions:

• 9-5 in office.

Pay: $23.00 - $30.00 per hour
