Lenawee County, MI is Hiring a Case Manager - MMAP (11220) Near Lenawee County, MI

Assess older person's situations and match services with identified needs; perform related work as required.

SUPERVISION RECEIVED: Works under the general direction of the Department on Aging Director.

SUPERVISION EXERCISED: Over those assigned to assist.

ESSENTIAL DUTIES AND RESPONSIBILITIES: An employee in this class may be called upon to do any or all of the following: (These examples do not include all of the tasks which an employee may be expected to perform).

  • Respond to telephone calls from older persons or caregivers of older persons who need assistance with services beyond the basic services offered by the Department on Aging.
  • Respond to persons who walk into the Department on Aging requesting assistance with services beyond the basic services offered by the Department on Aging.
  • Assess the needs of the older individual and make the appropriate referral for service.
  • Work with other department staff with concerns they may have for older persons they are working with.
  • Coordinate the MMAP program, following the MMAP, Inc. guidelines, including being a certified Counselor.
  • Represent the Department on Aging on various community committees, and other groups ans appropriate.
  • Speak to groups about Department on Aging services and other pertinent topics.
  • Respond to the Gate Keeper referrals from Consumer's Energy and R2AAA..
  • Perform other related duties as assigned.

DESIRABLE EDUCATION, EXPERIENCE and/or QUALIFICATIONS: An employee in this class, upon appointment, should have the equivalent of the following training and experience.

  • Graduation from an accredited high school plus two (2) years of college.
  • At least two (2) years experience working with older adults.
  • Must be able to problem solve in an efficient manner.
  • Must be interested in the welfare of the elderly population.
  • Have the ability to coordinate training's and programs as necessary.
  • Must be able to interact well with older persons, caregivers and co-workers.
  • Must be able to work independently.
  • Ability to manage volunteers.
  • Must have a thorough understanding of problems facing older persons in Lenawee County.
