JOB DESCRIPTION SUMMARY

Under general supervision, performs clinical and clerical functions in Personal Health Services working directly with clients in clinical, community and home settings. Works within scope of practice under established medical protocols and as directed by Physicians, advanced practice clinicians, and/or lead Registered Nurses. Maintains and prepares program reports, logs and data collection as assigned.

SUPERVISION: Work is performed under the supervision of the Nursing Director, Public Health Nurse Supervisor, Nurse Practitioner/Clinic Coordinator and/or a public health physician.

ESSENTIAL DUTIES AND RESPONSIBILITIES: An employee in this class may be called upon to do any or all of the following: (These examples do not include all of the tasks which an employee may be expected to perform).

  • Assists in community health education on a variety of topics and represents the department at various community activities and meetings related to health promotion and disease prevention.
  • Participates in planning, developing, implementing and coordinating personal health programs including providing direction to program staff.
  • Performs client assessment and provides services including but not limited to:
    • Obtaining vital signs.
    • Laboratory procedures including obtaining and preparing blood and other bodily fluid samples for submission to external laboratories.
    • Placement and reading of tuberculin skin test.
    • Providing immunizations and administering various medication (oral and injectable) within scope of practice to infants, children and adults.
    • Documenting care in the electronic medical record accurately and timely.
    • Provides client specific education, information and referrals to appropriate internal or community agency programs.
    • Establishes client care plan and provides case management.
    • Maintains efficient clinic flow, appropriate clinic supply and orderly work environment.
  • Ability to work independently as well as within a team care concept and to accept and utilize direction and supervision.
  • Evaluates clients for lice, scabies and performs other Public health nursing services when assigned.
  • Investigates monitors and reports communicable diseases, enters data in appropriate state databases.
  • Assists in training of new staff and Nursing or allied health professional students as assigned.
  • Assists other clinicians as needed.
  • Provides continuity of patient care by planning and exchanging information with other health and social agencies as appropriate.
  • Ability to lift safely with in a weight range of 0-45 pounds as well as transport equipment and supplies to offsite events.
  • Ability to work in multiple programs, flexibility in adapting to new program or changing program requirements.
  • Participates in the agency's Emergency Preparedness program as assigned.
  • Participates in occasional assigned community outreach occurring after regular work hours or weekends.
  • Performs related specialized assigned duties in the event of a public health emergency.
  • Assesses self needs for professional growth, participates in education/training that increases knowledge and skills in areas of responsibilities.
  • Adheres to established policies and procedures.
  • Maintains up to date vaccines including the influenza vaccine in accordance with CDC guidelines: Recommended Vaccines for HealthCare Workers.
  • Initiates, maintains and updates various forms and procedures, prepares state and departmental reports as assigned.
  • Performs other duties as assigned.

DESIRABLE QUALIFICATIONS FOR EMPLOYMENT:

  • Bi-lingual in Spanish preferred.
  • Two years of experience in a public health or community health setting.
  • Critical clinical assessment skills within the scope of practice.
  • Ability to instruct patients and families regarding basic health maintenance and simple nursing procedures.
  • Ability to establish and maintain effective working relationships with clients and their families.
  • Ability to work with culturally and socio-economically diverse populations.
  • Commitment to customer service skills, displaying discretion, patience and professionalism.
  • Strong written, verbal, interpersonal and organizational communication skills necessary to interact with all age groups, coworkers, state and other community officials and health care providers and the general public.
  • Ability to see, hear and speak effectively and without limitation.
  • Ability to stand or sit in one spot for long periods of time, moving arms, hand, back and entire body.
  • Ability to understand and follow oral and written instructions.
  • Considerable - computer literacy with ability to be trained on specific software programs.
  • Willingness to attend assigned seminars and meetings.
  • Willingness to continue required advanced education.

An employee in this class, upon appointment, should have the equivalent of the following training and experience:

  • License to practice as a Nurse in Michigan.
  • Valid driver's license and reliable transportation including proof of insurance.
  • Cell phone.
  • CPR certification.

Additional Information

Department: Lenawee County Health Department
Overtime Exempt: No
Union: Non-Union /Full-Time (37.50 hours per week, 1,950 hours per year)
Work Location: Lenawee County Health Department, client homes and community venues as required.
Pay Grade: NU7533 (Hourly rate listed at the top of this posting)
