LHC Group is Hiring a Bereavement coordinator Near Brookhaven, MS

Qualified professional who supervises the provision of bereavement services reflective of patient and family needs. Establishes a Plan of Care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery up to thirteen (13) months following the death of the patient.

Responsibilities

  • Demonstrates proficiency in providing bereavement services to persons of all races, ages, genders, sexual orientation, social class, spiritual beliefs, and mental / emotional status as evidenced by observation and EGSS scores.
  • Ensures all LHC-required Bereavement Mailings are sent on time either by the Bereavement Coordinator or by volunteers.
  • Demonstrates active listening and impactful suggestions to bereaved during phone and home visits within LHC-required time guidelines as evidenced by observation, Visit Notes and EGSS scores.
  • Leads and coordinates at least 12 effective sessions of Grief Support Group per calendar year as evidenced by observation and EGSS scores.
  • Coordinates with other Hospice Team members in creating at least one meaningful Memorial Service per calendar year as evidenced by observation and EGSS scores.
  • Creates and executes plans for at least one meaningful Holiday Event per calendar year as evidenced by observation and EGSS scores.
  • Maintains Bereavement Binder with information and attendance sheets from Memorial Services, Grief Support Groups, Holiday events, and other Hospice and Community groups and presentations.
  • Demonstrates individualized and compassionate care for each bereaved as evidenced by documentation in Visit Notes, Bereavement Risk Assessment, Care Plan, and EGSS scores.
  • Demonstrates knowledge of a wide range of bereavement resources including resources for children / teens, counseling services, brochures, online support groups and internet resources.
  • Demonstrates ability to assign and supervise Bereavement Support Volunteers.
  • Demonstrates ability to work efficiently and effectively within the Interdisciplinary Team and provides insight in the IDG meeting concerning the bereaved’s psychosocial and spiritual needs.
  • Demonstrates healthy self-care, furthers one’s own continuing education in their field and maintains healthy boundaries with the bereaved and their loved ones.
  • Provides bereavement support to Hospice staff as requested.
  • Provides educational and / or support programs for staff, facilities and community groups as requested.
  • Functions as a preceptor to new hires.
  • Participates in ongoing quality assessment and performance improvement for the Agency on a daily basis.
  • Fulfills all other duties as assigned by the agency Executive Director and / or Director of Bereavement and Chaplain Services.
  • Submits twelve hours of continuing education each calendar year related to the Hospice role to the Executive Director..

Qualifications

Education Desired

Bachelor's or Master's degree in Social Work, Psychology, Mental Health counseling, Psychiatric Nursing, Pastoral Counseling or the documented equivalent in education, training or experience per state regulations.

Experience Requirements

Experience in grief and loss counseling.

  • Excellent writing and presentation
  • Current CPR certification
  • Current driver's license, valid vehicle insurance and access to a dependable vehicle, or public transportation.
