LHH is assisting a local nonprofit in hiring a Case Manager! Our client located on the northside is seeking a dedicated Case Manager to assist their community members in areas of crisis counseling, education, and connection to resources. The ideal candidate for this role will have 1 years of Case Management experience and is open to a flexible schedule.
Summary:

• Hire type: Contract to Hire
• Location: 100% onsite, near Edgewater neighborhood
• Hours: Monday – Friday, standard business hours; some evening and weekends (rotation)

Responsibilities:

• Assist clients through crisis management, needs and service planning, and case management/advocacy
• Providing education and information on resources to clients including employment and health resources
• Assessing housing needs, assist client on securing housing and planning logistics
• Update and maintain client files, reports, and ensure proper guidance and support throughout program

Qualified candidates possess:

• At least 2 years of experience in case management, transitional housing, or crisis counseling
• Higher education preferred
• Must be compassionate and a strong advocate for our clients
• Must be passionate about social services
• Must be available for weekend and evening rotation

Pay Details: $21.00 to $24.00 per hour
