RESPONSIBILITIES:
- Collaborate with the team to create, monitor, and assist in the implementation of individualized care plans that align with clients' short- and long-term goals.
- Offer intentional case management to support clients in transitioning from high-intensity service levels to a more independent, recovery-focused level.
- Safely and courteously transport passengers and their personal possessions to their destination.
- Demonstrate a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs.
- Work effectively both independently and collaboratively within a team.
- Exhibit mature judgment, motivation, self-starting initiative, and proactivity.
- Excel in communication, whether in writing or verbally.
- Possess a strong sense of prioritization and can manage multiple demands in a high-pressure environment.
- Experience in homeless services, case management, mental health, and/or permanent supportive housing.
Pay Details: $21.00 to $23.00 per hour
