Outpatient Chemical DependencyPosition SummaryUnder the oversight of an LICDC-CS, educates patients and their families in individual and group settings; provides individual and group counseling; documents appropriately in the patient’s medical record; functions as a member of a multidisciplinary treatment team. This position requires the ability to work with an adult population of patients with psychiatric and substance use disorder diagnoses.

- Make referrals for other mental health services and community resources, to ensure appropriate level of care.
- Coordinates services with community providers.
- Serve as an advocate for patients with courts, schools, treatment centers, and other services as appropriate to make referrals and conduct follow-ups.
- Provide outreach and case management.
- Complete daily rounding
- Facilitate/co-facilitates therapy groups as assigned.
- Prepare written reports, regular case dictation and summaries for consultation according to program standards and professional ethics.
- Participate in curriculum development and supervision of clients.
- Work as part of a treatment team to coordinate care.
- Performs drug screens as needed.
- Intervenes in crises as needed.
- Transports patients as needed.
- Must have Chemical Dependency Counselor certification (LCDC III or LICDC).
- Must have knowledge and experience working with alcohol/drug assessment and case management.
- Must be able to form good working and therapeutic relationships.
- Must be organized, detail oriented, and the ability to maintain confidentiality.
- Must have competent oral, written, and interpersonal communication skills.
- Good working knowledge of computers and familiarity with Microsoft Office.
