Description
Job Summary: Within an Interdisciplinary Team (IDT) setting, responsible for promoting and maintaining health of LIFE Pittsburgh Participants in the community, through initial implementation of nursing care plans, providing nursing care, as well as periodic re-evaluations of Participant and family nursing needs. Conducts assessments, treatments, and provides teaching and counseling to Participants and family members. Will coordinate and mentor, center health aides and home care aides to support quality care delivery.
Essential Functions:

• Demonstrates and reflects a commitment to the mission and core values of LIFE Pittsburgh to support the elderly in maintaining their dignity and independence in the community.
• Take and monitor Participant's vital signs such as respiration, temperature, blood pressure, and heart rate
• Administer basic and personal care to include wound care/dressing changes, inserting catheters, assisting with respiratory treatments, and other as determined by skill/credential of the individual (to include basic wound care/dressing in the home setting)
• Report Participant's status to RN, PCP and/or CRNP
• Administer medication in center, pharmacy count verification, medication box preparation and home deliveries
• Perform basic diagnostic tests such as PT INR/Blood Glucose/urine dip test and report results to RN/CRNP/PCP
• Manage laboratory requisitions
• Assist with physician order process
• Provides services promptly, exercises tact, patience and courtesy at all times and conducts themselves in a professional manner with Participants, families, co-workers and any external contacts. As a representative of LIFE Pittsburgh, promotes goodwill and confidence in our Participants, staff, vendors, visitors and the general public.
• Proactively maintains requirements for the position (e.g., access of a vehicle, valid driver's license and auto insurance, annual TB, bi-annual physical, etc) and maintains compliance with educational requirements (e.g., Fire Safety, Infection Control, Annual Training, Safe Mobility).

Knowledge /Skills / Abilities:

• Ability to obtain and interpret information in terms of the specific needs of the Participant.
• Frequently required to manage many details within a fast paced environment
• Possesses knowledge of the Human Anatomy and development of the Participant served.
• Understands the range of treatment needed by the participant served.
• Well-developed organizational and interpersonal skills. Strong written and verbal communication skills are needed.
• Ability to multi task and problem solve complex situations
• Must be able to support cognitively impaired seniors and their families to deal with sensitive issues and facilitate problem solving.
• Must be flexible and able to change easily.
• Ability to work with people from various social- cultural backgrounds effectively
• Ability to work independently when appropriate and also work as a strong partner with a Multidisciplinary team

Required Certification/License:

• Valid PA Licensed Practical Nurse License
• Valid CPR and First Aid certification
• Valid Driver's license and automobile insurance
• Must have access to a vehicle

Education/Experience:

• Minimum 1 year experience working with the frail and elderly population.
