We are looking for a full time HOME CARE LPN to join a team of professionals, who excel in working in a unique health care program for Seniors! You have an essential role in conducting in home patient visits to perform a great assessment, execute the prescribed plan of care that includes performing lab draws, PEG tube care, catheter maintenance, wound care and administer medications, treatments and therapies. In addition, educate the patient or caregiver regarding the disease process, self-care techniques, and prevention strategies to ensure high-quality care.

Since this candidate will be working exclusively with seniors, some with dementia, patience and extra compassion is required. This position requires great listening skills and that you use to determine each patient’s needs, preferences and abilities. This position requires a high level of discipline with minimal supervision and requires excellent organization and time-management skills.

Requirements

Qualifications:

· Must be a licensed Registered Nurse (LPN) in the state of Oklahoma

· A minimum of one year of home care experience is preferred.

· A minimum of one year of geriatric experience is preferred.

· Possess a valid state driver’s license and automobile liability insurance, mileage paid.
