$18.75 - $19.75 USD Hourly

Veterans Engagement Specialist – CCBHC Grant – Posting #22-193
MAJOR DUTIES: The Veterans Engagement Specialist works under the supervision of the Program Coordinator. The specialist contacts the patients that are enrolled or referred to any of our behavioral health programs. The purpose of the contact is to identify barriers to the patient attending and addressing them to re-engage the patient. The specialist also helps coordinate trainings and resources about and for veterans. This position provides clinical services to agency patients. In providing these services the individual performs the following duties independently:

In providing these services the individual performs the following duties independently:
- Identify barriers to patients attending and addressing them (i.e., different day, time, etc.)
- If a patient cannot be re-engaged in treatment services, create Discharge/Transfer Planning for patient.
- Assess and evaluate patient needs, strengths, goals, and resources.
- Refer and link patients to other human services programs after a series of continued late cancels or patient no shows for appointment.
- Work with interdisciplinary team in reviewing cases and developing Individual Service Plans to reengage patients into services.
- Intervene, stabilize, and manage acute crisis situations.
- Provide linkage case management and participate in treatment planning.
- Consult with other agency staff regarding assessments and service plan development.
- Consult with other service agencies around case-specific and programmatic services.
- Conduct all professional activity in an ethical manner.
- Conform to all applicable policies and procedures.
- In conjunction with other staff assist in identifying and eliminating any situation which jeopardizes the health and/or safety of patients, staff, or others who come in contact with agency operations.
- Utilize literature, workshops, and other training aids to continually improve clinical and administrative skills.
- Assist in assuring compliance with all related licensing and accreditation requirements.
- Gather and provide veteran specific resources to staff.
- Assist with coordinating training around best practices for serving veterans.
- Provide community outreach to homeless veterans.
- Establish relationships with veteran serving agencies.
- Other responsibilities and tasks as may be assigned.
- Bachelor’s degree in human sciences or equivalent work or life experience required.
- Washington State SUDP credential preferred.
- 2 years of experience working with adults or youth with behavioral health disorders preferred.
- Good verbal and written communication skills.
- Great attendance.
- Ability to work well with others.
