**Description**

**LifeWorks** of Austin, TX is looking for a **Case Management Support Specialist** to join our **Workforce** team. The Education and Workforce Division is focused on developing the knowledge and skills needed to navigate the challenges of adolescence, move toward independence, and break the cycle of poverty. The Workforce division utilizes the Individual Placement and Support model of Supported Employment with young adults ages 16-26 who have experienced trauma. IPS refers to the evidence-based practice of supported employment which helps people living with behavioral health conditions gain employment of their choosing.

The Case Management Support Specialist tracks and manages the intake process for clients seeking program services and supports clients basic needs through service coordination in areas that include childcare and housing. Other services provided include supporting clients with the completion of intake process, administrative functions, as well as provision of basic needs, referrals, and crisis intervention. Through these services, youth and adults can achieve their educational goals, increase employability, gain independent living and parenting skills, and set themselves on a path to self-sufficiency .

This position earns a competitive rate and benefits, including 3 affordable health insurance plans (including an option that is no cost to the employee!), 2 dental plans, and a vision plan. ****RESPONSIBILITES****

* Provide case aide services and coordinate services with other agencies to assist clients in overcoming barriers that inhibit success in education, employment, or training.

* Partner with Employment Specialists, other internal program staff, and partner agencies to assist clients with their.

* Collaborate with partner agencies to address clients individualized needs.

* Complete intake forms and assessments daily with clients seeking Workforce program services.

* Provide referrals for counseling, education, housing, and any other support interventions the client needs and/or desires.

* Provide individualized services to clients including but not limited to transportation to appointments, completing paperwork or online applications, and working on clients goals.

* Effectively manage crisis situations through assessing and acting in accordance with standards and laws in emergency situations.

* Perform administrative functions; entering and tracking client intake and interactions, client file maintenance and ensuring accuracy of data to maintain compliance.

* Practice professional boundaries with clients and partners to maintain positive relationships.

* Advocate for clients within the community as well as internally to ensure that client needs are being recognized, represented, and addressed.

****QUALIFICATIONS****

**EDUCATION AND EXPERIENCE**

* Requires a Bachelors Degree in social or behavior sciences or administrative field;

* Requires a minimum of 6 months experience with youth; or

* Equivalent combination of education and experience.

* Proficient in HMIS, Microsoft Office, Outlook, and agency database.

**CERTIFICATES, LICENSES, REGISTRATIONS**

* Must possess or be able to obtain a valid Texas driver's license within 30 days and provide proof of valid insurance.

* Standard First Aid and CPR Certification as required by individual program.

**PHYSICAL/ENVIRONMENTAL REQUIREMENTS**

* Frequently required to sit or stand, use manual dexterity, speak, listen, hear, and write.

* Works in an office or indoor environment with little or occasional light physical effort, and moderate noise.

* May occasionally lift and/or move up to 25 pounds and occasionally lift to 50 pounds.

* Up to local metro travel required, including transporting clients, participating in community outreach programs, attending meeting and training opportunities.
