Description

Lifeworks is seeking an FMS Service Coordinator to assist people with disabilities and/or their families to lead person-centered, self-directed lives by utilizing their funding to purchase services that meet their needs and life priorities. Facilitate and manage changes related to the individual’s service plan, budget, and service authorization.

Position Title: FMS Service Coordinator

Department: Fiscal Services

Hours: Full-time; 40 hours/week, Monday – Friday (Exempt)

Location: Lifeworks Services – Richfield / Hybrid

Compensation: $47,500/yr

You’ll succeed in this role if you:

· Strong analytical and financial acuity skills

· Strong communication skills

· Thorough understanding of disability services and funding sources available in Minnesota

· Commitment to person-centered service delivery using effective negotiation skills.

· Case management experience related to Fiscal Support Entity services and person-centered facilitation.

How you’ll be spending your time:

· Ensure the support plan is based off the county and/or state Authorization for Services, and ensure an accurate final authorization is received. Facilitate and manage changes related to the person’s service plan, budget, and service authorization. 

· Coordinate services for assigned caseload including scheduling, organizing, participating in, and documenting all requirements per MN 245D and 256B license or Lifeworks practices by collaborating with the Expanded Support Team and others as appropriate. 

· Conduct meetings or site visits as required per service line; coordinate with support staff to ensure services provided meet the expectations of the person, the person’s expanded support team and other interested parties. 

· Audit the service plan and/or person’s file as needed to ensure quality standards and licensing (i.e., 245D, 256B) requirements are met. 

· Ensure systems are current with all contact information and method of contact preferred. 

· Conduct intake and renewal meetings and ensure that all required documents are complete. Provide instruction to persons and families in completing documentation and forms as requested for continued access to services and funding.

· Participate in cross-functional process improvement initiatives and projects to improve the delivery of Self-Directed Services. 

· Keep informed of changes in federal, state, and county statute requirements, licensing, and funding requirements, as well as Lifeworks’ service adjustments to ensure competent coordination of services.

· Work collaboratively with Lifeworks staff to implement changes in statute and licensing requirements to ensure compliance across all Self-Directed Services.

· Work with leadership and finance department to ensure services are being delivered within the individual’s service authorization to prevent service disruption and uncompensated care. When required, represent Lifeworks at industry associations and government meetings. 

· Follow safety procedures and assist in identifying safety needs for self and others. Contribute to the organizations mission by complying with Lifework’s policies and procedures, using resources wisely and performing other duties that may be assigned as necessary.

Requirements

Qualifications:

Required:

· A four-year degree in Human Services, Human Development, Psychology, Liberal Arts, or a related field; plus, a minimum of 2 year of experience in human services field. 

· 20 – 35% travel as required to interact with persons, their families, county personnel and external training. 

· Must possess a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes.

· Strong computer skills and knowledge of Microsoft Suite and CRM Database

· Proficiency with a second language desirable, preferably Spanish, Hmong, or Somali.

· Have reliable transportation to work and clean driving record.

Preferred:

· Meet Designated Coordinator qualifications

· Candidate must complete and pass a pre-employment background study; a physical and drug screen; and have access to reliable transportation for work purposes.
