Job Summary:

LINC is seeking a dedicated Case Manager with a strong background in human services to provide comprehensive interventions and resources related to housing in the Tri-County area. In this Case Manager, your crucial role is to address the living conditions that directly impact housing stability for program participants. You will focus on providing supportive services to help individuals and families maintain housing and achieve personalized goals.

LINC's Permanent Supportive Housing Program (PSH) relies heavily on case managers to strengthen and support individuals residing in permanent supportive housing. Case manager representation of LINC should always be friendly and professional. Your commitment to empowering program participants and promoting safety within the community is crucial. Your work will be guided by a strengths-based perspective, emphasizing collaboration and coordination across various settings, including homes and the broader community.

Key Responsibilities:

  • Work closely with formerly homeless clients who are primarily chronically homeless and are living in scattered-site permanent supportive housing.
  • Address clients’ health, safety, and welfare needs while ensuring housing stability
  • Utilize evidence-based practices, including a Housing First philosophy, Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
  • Establish and maintain positive professional relationships with clients, landlords, service providers, and community partners.
  • Accurately record and maintain client data, including demographics, outcome measures, goals, face-to-face notes, medical information, and grant-specific requirements.
  • Uphold confidentiality and security standards.
  • Work collaboratively with other Case Managers to ensure continuity of care.
  • Attending team meetings, agency-wide meetings, and relevant training.
  • Utilize a strengths-based perspective to manage and coordinate programs and services.
  • Support staff in executing programs within the home, school, and community to meet identified needs.

Qualifications:

  • Extensive knowledge of case management, housing programs, and documentation.
  • Familiarity with community housing initiatives and resources.
  • Experience working with formerly incarcerated or marginalized populations.
  • Lived experience with mental health conditions, substance abuse, or trauma is beneficial.
  • Availability for on-call emergencies, weekends, and public holidays.

Other Requirements:

  • Valid driver’s license, copy of driving record, and proof of valid auto insurance.
  • ·Availability during some evenings and weekends.
  • ·Physical ability to move, climb stairs, and engage in outside client contact regularly.
  • ·May require local travel within the tri-county area.
  • ·Mandated reporting of suspected abuse or neglect as required by law.
  • ·Willingness to perform other necessary tasks as assigned.

Preferred Requirements:

  • Alternatively, an equivalent combination of training and experience enables successful performance of the essential duties listed above.
  • One (1) year of experience providing case management and/or treatment services in similar settings.
  • Bachelor’s Degree in Psychology, Social Work, or an equivalent field, or an Associate’s Degree in a Human Services field.
  • High School Diploma with two to five (2-5) years of work experience in culturally sensitive environments and/or workforce program settings.
  • Proficiency in communication and customer service, including reflective listening skills to identify needs and rephrase information for clarification.
