Our Staff Deliver Intensive Culturally Responsive, Trauma-informed Home-based Supports Through Four Phases Of Service
Family Engagement: Identifying the family’s strengths and cultural experiences to determine together specific community support needs;
Goal Setting: Working together to develop and implement an individualized plan of care designed to meet the goals set by the youth and family;
Plan Implementation: Supporting the family in driving their plan and effectively meeting their goals;
Transition: Celebrating the family’s success and strengthening links to identified community supports for sustained stability and permanence.
Youth who have been in foster care, the juvenile justice system – or both – typically face great challenges as they transition back to their families. Many have experienced trauma and have been completely removed from family and their community, which has a significant impact on their ability to trust and have healthy relationships. Repairing and building relationships within the family and community is critical to well-being and a successful transition to a stable and permanent family environment. POSITION SUMMARY: Reporting to the Community Liaison Supervisor, the Community Liaison teams with the Clinician, Family Partner, and family to develop a wraparound plan to support children and youth on probation or in the foster care system to live in the least restrictive environment possible and to address child’s social, recreational, spiritual, and life-skill development. Essential Functions
Provides one to one support to youth in the community, home and school setting. Services provided are based on the Wraparound Philosophy of Treatment.
Collaborates with the team’s Family Partner and Clinician to assess client needs utilizing a strengths-based perspective.
Works with youth, caregiver, community supports, Family Partner, and Clinician to create goals to support youth in obtaining a stable permanent placement.
Implements the goals set by the Family Team; follows interventions established on client’s treatment and wraparound plan when meeting with individual clients.
Provides Individual Rehab Services, Case Management Services and Crisis Management Services to children and youth with the purpose of helping them meet their treatment goals. Coordinate with collateral contacts to coordinate service provision.
May participate in rotation of staff responsible for answering the on-call crisis phone 24/7 for one week at a time.
Collaborate with Project Permanence Team, client, family and community supports on a monthly basis to re-assess goals and create a transition plan for youth who are exiting the program.
Complete necessary documentation as required by Medi-Cal, Alameda County Behavioral Health Care Service and/or Lincoln.
Participate in weekly supervision and in external and in-service trainings relevant to the program services.
Requires driving to clients at various locations across Alameda County and surrounding areas to conduct meetings at homes, schools or other sites and transporting clients to/from court appointments. (Must have valid CA driver license, availability of personal vehicle, personal automobile insurance and satisfactory driving record.)
POSITION REQUIREMENTS: Demonstrated awareness of, sensitivity to and competence in dealing with cultural and socioeconomic diversity of client population. Ability to manage time independently and demonstrate strong relationship-building skills. Willingness and ability to be trained using our therapeutic and relationship-building philosophy that includes Collaborative Problem-Solving, Life Space Crisis Intervention (LSCI), and SAFE Environments training programs. Must be able to work a flexible schedule based on program and client needs which generally is weekdays from 10:00 AM to 6:00 PM and weekends as needed. Experience And Education
Bachelor’s Degree in related field (e.g., psychology, sociology, social work) preferred; high school diploma or equivalent required.
6 months of experience providing wrap-around or other community-based collaborative services to children and their families, including participation in Child and Family Teams, is preferred.
Experience with children, including experience in clinical interventions, setting limits, and crisis intervention is required.
