Summary/Objective
The MDS (Minimum Data Set) Coordinator has a heart for older persons and is passionate about enhancing the quality of life and care for those at St. Joseph’s Home for the Aged – Louisville, KY.. As an integral member of the nursing team, the MDS Coordinator ensures the accurate picture of the resident managing all aspects of the MDS assessment and care planning process in accordance with current federal, state, and local standards alongside the mission, vision, and values of the Little Sisters of the Poor.
This position intentionally advances person-centered care and services irrespective of the presence of chronic and/or acute disease. These standards and guidelines are designed to attain the highest possible degree of quality of life and quality of care across multi-levels of aging services; ranging from residential to skilled nursing. St. Joseph’s Home for the Aged – Louisville, KY. is licensed for 35 skilled nursing beds to complete the senior living community.
Requirements and Qualifications:
· Active license with the state as a Registered Nurse in good standing.
· Minimum of an Associate Degree in Nursing and 3 years of long-term care experience in an MDS Coordinator role, preferably certified in Resident Assessment Coordination (RAC-CT).
· Medicaid & Medicare program experience preferred.
· Proficiency in computer usage and software applications, including, but not limited to, electronic medical records and Microsoft Office.
· Ability to follow and give written and oral directions. Proficient in English.
· Ability to work both independently and collaboratively.
· Communicate effectively by dealing tactfully and respectfully with all persons.
· Flexibility and a willingness to adapt to change in an ever-evolving environment.
· Ability to demonstrate continual learning skills, effect changes in approach to care based on established, evidence-based practice.
· Possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, polices and procedures, etc., that are necessary for ensuring the accurate and timely completion of the RAI documents.
Essential Functions: Manuals, Policies/Procedures and Mission Integration materials for the Little Sisters are located in the Director of Nursing Services Office. A personal copy of current Employee Handbook, the Home’s Supplement and Benefit Guide are provided at time of hire.
1. Gather the MDS (Minimum Data Set) via the Resident Assessment Instrument (RAI) process, which assesses a resident’s behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized care plan, including measurable objectives and time tables, that considers the resident holistically. The MDS Coordinator then leads the interdisciplinary team in developing a comprehensive resident assessment and care plan for each resident, ensuring care plan guidelines for each resident are followed and compliant with ethical standards and Medicaid and Medicare requirements.
5. Assure effective communication and education with the interdisciplinary team as it relates to documentation guidelines and regulatory requirements, including attending physicians and nursing staff.
6. Work closely with hospital discharge planners and physicians to obtain accurate and complete documentation to support ICD-10-CM diagnosis coding.
7. Support timely, accurate, and complete documentation of clinical information.
8. Complete/submit PASRR documents and ensure appropriate follow-up.
9. Be highly involved in determining skilled level of care for Medicare residents and procuring required Medicare-specific documentation; be responsible for physician certification/recertification of a skilled level of care throughout the Medicare stay; be involved in making Medicare eligibility determinations.
10. Coordinate care with case managers for residents utilizing managed care, health maintenance organizations (HMOs), commercial insurance, and other alternate payment models.
11. Actively participate in case mix audits and in Facility surveys (inspections) made by authorized government agencies.
12. Access and provide Quality Measures, CASPER, and Case Mix reports as requested.
13. Participate in the monthly Triple Check meeting; prepared with necessary reports in order to complete a final check or audit to ensure all requirements of Medicare billing are in place prior to submission of the claims.
