Little Village Nursing and Rehabilitation Center -

Reports to : Director of Nursing

  • Purpose: Supervise day to day C.N.A. compliance with overall restorative nursing functions and programming.
  • Ensure/ Supervise that C.N.A.'s and nurses are accurately completing required documentation necessary to assess resident needs and functional level within required time frames.
  • Ensure/ Supervise that C.N.A.'s are providing care planned restorative programming services.
  • Complete accurate and timely clinical record documentation per facility policy and State/ Federal guidelines.
  • Plans, directs and evaluates total restorative nursing care of residents.
  • Assists the facility in implementation of policies and procedures as it relates to restorative nursing care and reporting of resident condition, and level of assistance needed to ensure appropriate service delivery, offer accurate documentation on the MDS and State and Federal reimbursement requirements; coordinates determining Assessment Reference Dates to ensure appropriate RUG scores.
  • Provide C.N.A. and nursing training to ensure accuracy in reporting resident condition and sevices for the MDS, in accordance with Federal and State regulations.

Qualifications & Essential Requirements :

* Current and unrestricted active R.N/L.P.N. Illinois State License preferably with Rehab Certification and Licensed in the State of Illinois.

* Must be able to read, write, speak and understand the English langauge.

* Must be able to communicate effectively with all members of the care plan team, including training and supervision of C.N.A.'s regarding implementation of restorative programs.

* Ability to make independent decisions when circumstances warrant such action

* Must possess ability to deal tactfully with personnel, residents, family members, visitors, goverment agencies/ personnel and the general public.

* Must be knowledgeable of nursing and medical practices and procedures, as well as laws and regulations and guidelines that pertain to long term care.

* Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.

* Must be physically capable of effectively hearing , seeing and speaking.

* Must be physically capable of walking, prolonged standing, taking stairs, running, prolonged sitting, bending, pulling/pushing weights exceeding 150lbs and fine motor dexterity (computer usage).

* Must be able to physically and emotionally assist in the evaluation of residents during emergency situations.

* Experience in Medicare and State reimbursed services, including documentation requirements of skilled services.

Duties/ Responsibilities/Function (Every effeort has been made to identify the essential duties of this position.   1. The assessment, care plan development and management of the treatment plan of all Restorative Nursing Programs in conjucntion with other members of the interdisciplinary team.
  2. Determination through on-going assessment of the residents' functional capabilities the need for changes to resident care plans to prevent decline and promote resident safety.
  3. Attending weekly Medicare Meetings and Care Plan Conferences.
  4. Coordinates with the Interdisciplinary Team to ensure appropriate programming to address restorative needs (i.e. fal intervention, splint and adaptive equipment usage).
  5. Directing C.N.A.'s to ensure compliance with all elements pf the nursing restorative program.
  6. Ensure that all required restorative program documentation is accurately completed in a timely manner.
  7. Assess residents to determine restorative program needs in conjunction with the IDT.
  8. Monitor and direct C.N.A. provision of restorative plans of care.
  9. Evaluate /assess all admits/re-admits within 24 hours to determine restorative/rehab needs.
  10. Monitor and ensure that C.N.A.'s are maintaining all restorative program protocols. Provide training, inservicing and disciplinary actions as warranted and deemed appropriate.
  11. Participate and developin-services for all satff regarding restorative and rehab programming, treatment and clinical record documentation as necessary.
  12. Ensure that all emergency and disaster policies and procedures are maintained and enforced.
  13. Ensure that appropriate documentation/ charting is completed as required and in accordance with established policies and procedures.
  14. Ensure the resident environment is as free from accidents and hazards as is possible. Immediately correct/address potential safety risks as prudent and warranted.
  15. Participate in C.N.A. performance evaluations as directed by ADON/DON.
  16. Ensure compliance with infection control standards. Immediately correct identified instances of non compliance.
  17. Ensure that elements of the facility's Abuse& Neglect policies are maintained and enforced.
  18. Ensures HIPPA compliance is maintained at all times.
  19. Strictly safeguards computer passwords and computer use authorities. Understands the importance of logging on and off the computer clinical record system and will consistently maintain all computer protocols.
  20. All other duties assigned.
