Livingston HealthCare is Hiring a Community Healthcare Worker Near Livingston, MT

JOB SUMMARY:

**This is a grant-funded position for 2 years. Position contingent upon additional funding**

The CHW provides outreach to disproportionately affected community members with an effective, individualized, and culturally tailored approach to improve health. The CHW is a frontline health worker who performs a variety of duties at the paraprofessional level to engage, advocate, and educate specified community members or groups in overall health and wellness. The CHW serves as a liaison between the healthcare system and the community to facilitate access to care and improve health outcomes.

Schedule:

    1.0FTE (40 hours)
    Mon-Fri
    8a-4:30p

ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES:

  • Provides patient outreach with community resources on preventative care, disease management, and facilitate access to healthcare
  • Creates care coordination plans in collaboration with healthcare team
  • Assists patients in understanding care plans, instructions, and completion of forms and paperwork such insurance or financial assistance
  • Coordinates and monitors care to promote quality and cost-effective outcomes
  • Addresses health disparities at the individual and community level by identifying social determinants of health and health equity barriers
  • Document and communicate activities around key performance indicators (KPIs)
  • Local travel to meet patients in designated location/office

ADDITIONAL RESPONSIBILTIES:

  • Complete education training as required
  • Rapidly adjust and adapt to evolving role

QUALIFICATIONS (Required):

  • High School Diploma or equivalent
  • Completion of Community Health Worker Training within 6 months of hiring
  • Education and/or experience with peer counseling, health education, or community organizing
  • Knowledge of and/or part of community served
  • Information technology skills such as: laptop, smartphones, internet/online application systems
  • Demonstrate strong communication and collaboration skills
  • Ability to maintain confidentiality
  • Drivers license and insurance

ADDITIONAL DESIRABLE QUALIFICATIONS:

  • Previous healthcare experience and knowledge preferred
  • Demonstrate empathy, respect and advocacy
  • Flexibility in rapidly changing environment
  • Well-organized, detail-oriented and have the ability to multi-task in demanding and constantly changing environment
  • Ability to work independently and as part of a team
