Position: D964 Licensed Practical Nurse
The Licensed Practical Nurse manages and controls all phases of resident medications, treatment, and care as assigned. Supervises the direct care provided by the care team in the assigned unit and ensures residents are treated with dignity and kindness. The Licensed Practical Nurse must understandandimplementthecorporatemissionandadministerspoliciesandproceduresinsuchfashiontopromotethedesiredbusinessimageof thecommunitytoresidents,guest,andstaffmembers.Ensurescompliancewithcurrentfederal,stateandlocalstandards,guidelines,andregulations governingthefacility.TheMedicationLicensedNursepositionrequiresacaringnatureandwillingnesstocarefortheelderly.
Provides status reports to incoming nursing staff and obtains reports from outgoing nursing staff
Chartsandmaintainsmedicalchartsand/orprogressnotesallpertinentresidentinformationincludingbutnotlimitedtoadmissions,discharges and transfers, Change of Condition, notifications to family, health practitioners, family/responsible parties, case managers, pharmacies and all other communication related to residentcare
Completes periodic reevaluation of resident ADL assistance needs, SelfMedication Evaluations, and other reevaluations as scheduled Completes Incident/Accident report for resident and/or personnel before leaving the shift
Passesmorning,afternoon,andeveningmedicationsstayingwitheachresidentuntilthemedicationisconsumed,andchartsandexplainsall variances in the residentrecord
Reorders, receives, records, and/or restocks all medications from the pharmacy
CompletesthecountingofControlledSubstancesbeforeacceptingtheMedicationCartKeysandbeforehandingtheMedicationCartKeysoffto the oncomingnurse
Assists in all care of residents as needed
Provides direct supervision of care provided by care team
Maintains accurate, complete, and confidential resident medication and care records according to established procedures
Ensuressafeandcomfortableaccommodationsforresidentsandrespondstoinquiriesforinformationorassistanceinapromptandcourteous manner
Communicatesasneededwithdoctors,families,andotherappropriateparties,concerningresidentstatus,changeinstatus,needforan appointment and other relatedneeds
Develops and maintains an excellent working relationship with staff, interdepartment personnel, as well as with other departments and facilities within the community
Createsandmaintainsanexcellentatmosphereofwarmth,personalinterest,andpositiveemphasis,aswellasacalmenvironment Other duties asassigned
EXPERIENCE/EDUCATION
Musthaveappropriatenursinglicensure(i.e.,RegisteredNurse(RN),LicensedVocationalNurse(LVN),LicensedPracticalNurse(LPN),etc.). License must be active and in goodstanding
Minimum six months on the job experience as a licensed nurse required; experience providing care to seniors preferred Receives and maintains CPR and all other required certification(s)
Supervisory/administrative experience preferred
KNOWLEDGE, SKILLS & ABILITIES
Proficiency in English required
Proficiency in Point Click Care/electronic health record and electronic medication administration
Must be able to easily lift and carry 50 lbs.
