DescriptionLong Island Select Healthcare Inc. (LISH) is a comprehensive network of community health centers under a Federally Qualified Health Center (FQHC) model. We provide Article 16 and 28 clinic services at 8 locations across Suffolk County, New York.LISH is currently searching for a Full-Time Lead Nurse. We are seeking a positive, energetic, detail oriented individual who will be responsible for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practices the continuum of care to meet the needs of the patient and family.
General Duties Include
- Manages coverage for providers for Nursing call outs
- Receives and manages telephone calls
- Runs and participates in daily huddles and manage review schedules
- Manages and maintains waiting room, office and exam rooms, as needed
- Monthly inspections of all emergency equipment and supplies including expiration dates
- Performs duties as assigned by the Chief Medical Officer to support and ensure general medical operation of LISH
- Will provide relevant information about the PCMH program including brochures, that describe the valve and goals of the program to patients. The information disseminated will be based upon the person's role as it relates to patient care within LISH.
- Participates in maintaining safety of work environment
- Maintains office supplies and inventory and orders all medical and lab supplies.
- Provides oversight for Quality Management and Infection Control at dedicated sites.
- Adheres to agency's policies, procedures, rules and regulations of confidentiality
