DEPARTMENT SUMMARY: As the nation’s largest and most experienced provider of LGBT medical and mental health services, we help LGBTQ people lead healthier, happier lives, by keeping their minds and body well. All services are free or low cost. We’re also one of the few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research team working to advance the care and treatment of people in our community.
JOB SUMMARY: The Nurse Practitioner for Urgent Care and Coverage provides direct patient care for patients whose primary care provider (PCP) is unavailable. This position plays a vital and varied role supporting an outpatient-only primary care practice specializing in LGBT primary care, HIV medicine, Transgender healthcare and LBQ women’s care. In collaboration with a care team consisting of RN, LVN, MSW and MA support, this position provides urgent care, routine primary care during PCP absences, and fills in with other clinical duties as needed. The NP working under this Job Description does not have their own assigned panel of patients for ongoing continuity care.
ESSENTIAL FUNCTIONS:
• Conducts clinical encounters with cross-coverage and urgent care/same day patients including: taking an HPI (history of present illness); completing physical exams; documenting pertinent history; ordering diagnostic tests and interpreting results; developing differential diagnoses; documenting plans of care; and coding visits appropriately for billing purposes.
• Provides necessary referrals for specialty care according to protocols and standards as required by payors or as developed by the Center; reviews consults notes and speaks with consultants directly as desirable or requested.
• Demonstrates chart hygiene by keeping patient problem lists up to date and reconciling medications.
• Using the electronic medical record, follows practice protocols in documenting and signing clinical notes, clinically significant lab results and imaging reports in a detailed and timely a timely manner.
• Responds to refill requests and patient messages in a timely manner.
• Provides patient care that is evidence-based and in line with national standards and internal practice guidelines.
• Prescribes medications to the extent allowable by state guidelines, clinic regulations and established best practices.
• Participates in peer review exercises and completes mandatory clinical trainings.
• Seeks consultation from staff physicians as needed, especially when a patient’s problems are especially complex or if the patient’s condition is not responding as expected.
• Attends department and staff meetings as appropriate including (provider meeting, MDCC, care team meetings, staff meetings).
• Maintains sufficient Continuing Education Units (CEU) as required by the California Board of Nursing for license renewals and as may be required to maintain any certifications necessary to demonstrate proficiency in a specialty area relevant to NPs clinical practice.
• Other duties as assigned.
JOB QUALIFICATIONS AND EXPERIENCE:
• Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
• A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
• Completion of nurse practitioner training from an accredited institution and a license to practice in the state of California.
• Minimum 2 years clinical experience.
• Current DEA license and BLS certification.
• Clinical experience with HIV, STIs and sexual and reproductive health preferred.
• Commitment to providing care that is trauma-informed, sex and body positive, and harm reductive.
• Passion for working with the LGBT community health.
• Ability to work effectively with diverse populations.
• Ability to work efficiently in multitasking environment with competing priorities and complete tasks with a high degree of accuracy.
• Ability to work and solve problems independently.
• Willingness and ability to work evenings.
• Strong interpersonal communication skills, as well as written and verbal skills.
• Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
