Schedule:

Once you complete your orientation, your schedule as a Palliative Care Clinic Coordinator will typically be Monday to Friday, 8:00 AM to 5:00 PM, but will need to be flexible based on clinic needs.

What you’ll be doing:

  • We provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.
  • As a Palliative Care Operations Coordinator, you’ll collaborate and coordinate with external medical staff and referral sources, as well as our own clinical teams for continuity of care.
  • You’ll be assisting field staff with, or independently completing, prior authorizations for medications prescribed by palliative care.
  • Along with clinic duties, you’ll be calling patients for phone triage, or other patient questions or concerns.
  • You’ll be scheduling appointments, processing payments, patient appointment reminder calls,
  • Check in, check out, processing payments, entering in information in EMR
  • As the first point of contact for our patients you will provide exceptional customer service and ensure a smooth check-in and check-out process.
  • Working with other departments internally, like finance, patient access
  • Responsible for conducting Person-Centered Patient Assessments, which would include things like health education, and self-advocacy skills.
  • You’ll also coordinate general office administration tasks.
  • You will be part of an Interdisciplinary Team (IDT) consisting of a physician, nurse practitioner, registered nurse, and medical social worker.

Job Specifications

1. Education: College degree required; bachelor’s degree preferred.

2. Licensure / Certification: Patient Navigator Certification required within 3 months of hire.

3. Experience : Minimum of two years’ experience in a healthcare office setting.
