As a non-paid Case Manager- Intern, you will play a crucial role in supporting the overall mission of our organization by assisting in the coordination and implementation of comprehensive case management services. Under the guidance of the Senior Case Manager, you will have the opportunity to develop valuable skills in client advocacy, resource navigation, and program administration. Conduct initial assessments of clients to identify their needs, strengths, and challenges. Participates actively in citizen advisory council and Board of Directors meetings to gain insights into community needs and develop strategies for effective advocacy on behalf of the population. Collaborate with clients to set realistic and achievable goals. Assist in the development and implementation of individualized case plans for clients. Regularly monitor and evaluate progress toward client goals. Provide guidance and support to clients in accessing and navigating community resources. Advocate for clients’ needs and rights within the organization and in the broader community. Collaborate with external agencies and service providers to ensure clients receive appropriate support. Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with organizational policies. Prepare reports and documentation as required by funding sources and organizational standards. Communicate effectively with clients, colleagues, and external partners. Participate in team meetings and case conferences to discuss client progress and challenges. Participate in training opportunities provided by the organization to enhance skills and knowledge in case management. Engage in ongoing professional development to stay informed about best practices in the field. Assist in administrative tasks related to case management, including scheduling appointments, maintaining records, and organizing resources. Performs other such duties as may be assigned. Knowledge of community resources, of case management delivery systems, and of program policies and procedures. Skill in developing, implementing, and monitoring service and treatment plans. Ability to assess client’s needs, to coordinate clients’ services, to communicate effectively, and to train others. Skill in the use of a computer and applicable software. Ability to read, write, and speak both English and Spanish. Currently enrolled in a relevant undergraduate or graduate program (e.g., Social Work, Psychology, Counseling). Valid/Current Texas Driver's License.
