LTC Physician Care, PC is a new and emerging medical group dedicated to serving patients by providing Primary Care Physician (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships.

 LTC is seeking a dedicated Chronic Care Management Medical Assistant to join our team.

Job Structure

• This is a hybrid position. 
• Act as a liaison between patients and healthcare providers
• Schedule and conduct regular follow-up calls with patients to monitor their health status, adherence to care plans, and overall well-being 
•  Provide patients with education on managing chronic conditions, medications and lifestyle changes
• Assist in the development and implementation of individualized care plans for patients with chronic illnesses
• Document patient interactions, health data, and changes in electronic health records (EHR)
• Collect and review patient health information, including medical histories and current medications
• Support remote patient monitoring initiatives, such as tracking vital signs and symptoms 
•  Coordinate with pharmacists, specialists and other healthcare professionals to ensure comprehensive patient care
• Manage patient records and ensure all documentation is accurate and up-to-date
• Handle patient inquiries and provide information on available resources and support services
• Assist in the organization and scheduling of telehealth appointments
• Outstanding communication skills
• Ensure patients understand their treatment plans and any follow-up instructions

Requirements and Qualifications

• Minimum of 2 years' experience in a clinical setting, preferably in chronic care management'
• Strong understanding of chronic diseases such as diabetes, hypertension, COPD, and heart disease
• Proficient in electronic medical records 
• Ability to work independently and manage time effectively
• Compassionate, patient-centered approach to healthcare 
• Candidates must have completed a valid U.S. MA certification program
• Candidates must be able to satisfactorily complete a pre-employment background screening process (Criminal History, Licensing, etc.)
• Candidates must pass a pre-employment drug-screen 

 Compensation and Benefits

• Competitive compensation package:
• $18-$22 per hour 
• 15 PTO days annually
• $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance
• Monthly phone stipend
