LTC Physician Care, PC is a new and emerging medical group dedicated to serving patients by providing Primary Care Physician (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships.

LTC is seeking a dedicated Registered Nurse to join our team as a Chronic Care Management Specialist.

This position is Hybrid – Administrative position

Requirements and Qualifications

• Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited institution.
• Current RN licensure in the State of Michigan.
• Certification in Chronic Care Management (CCM) preferred.
• Minimum of 2-3 years of clinical experience in chronic disease management, primary care, or related healthcare settings.
• Strong assessment, communication, and patient education skills.
• Knowledge of evidence-based practices, clinical guidelines, and treatment protocols for chronic conditions.
• Proficiency in utilizing electronic health record (EHR) systems and care management software.

Compensation and Benefits 

• $35 - $50 per hour
• 15 PTO days annually
